Filipino nurses and doctors to UK, Ireland, EU, US (2026)
How Filipino-trained health professionals register in the UK, Ireland, Germany, the Netherlands and the USA: the documents to get at home, the exams you can sit in the Philippines and the rules that apply to you.
Overview
If you trained as a nurse, doctor, physiotherapist, radiographer or other health professional in the Philippines, every destination we cover has a route open to you, and your Philippine qualification is the starting point for each one.
The UK, Ireland, Germany, the Netherlands and the USA all assess the training you completed in the Philippines and the licence the Professional Regulation Commission (PRC) gave you, then add their own English or local-language test and their own exam or assessment. No destination registers you on the strength of a PRC licence alone, and none of them treats the Philippines as an English-speaking country for exemption purposes, so plan for IELTS or OET even though you trained in English.
Most people who leave are nurses. The UK has a long-standing government-to-government relationship with the Philippines for nurse recruitment: GOV.UK's code of practice says a new UK-Philippines memorandum of understanding was signed in October 2021 and grades the Philippines green, which means active recruitment is permitted. The USA is the other large destination: the NCLEX-RN can be taken at Pearson VUE centres in the Philippines, and professional nurses are a Schedule A occupation for the employment-based green card. Ireland, Germany and the Netherlands recruit Filipino nurses too, but Germany and the Netherlands require you to learn the local language to a professional level first.
What is different when you trained in the Philippines is mainly paperwork and timing. Your licence, board rating and good standing come from the PRC, which issues them through its online system. Your school records come from your college or university. Documents for Germany usually need an apostille from the Department of Foreign Affairs (DFA), which the Philippines has issued since the Apostille Convention entered into force for it on 14 May 2019, while the Dutch BIG register does not accept an apostille as certification at all. Each destination also sets its own validity window for good standing certificates, from three months (GMC, Irish Medical Council, BIG register) to six months (NMBI).
Many Filipino professionals also work in the Gulf or elsewhere before moving on. If that is you, every licence you have held matters: the GMC and Irish Medical Council want good standing from every regulator you were registered with in the last five years, and TruMerit's VisaScreen validates every licence you have ever held. Our guide for professionals licensed in the Gulf covers those extra steps.
This page sets out, destination by destination, what to do from the Philippines, which documents to collect before you start, which exams you can sit at home and which you must travel for, and what the international recruitment rules mean for you. Fees and scores are taken from the regulators' own pages and are linked in the sources; check them again before you pay, because they change.
Where you can go
United Kingdom
The most established route for Filipino nurses, under a government-to-government memorandum of understanding and a green grading in the UK code of practice. Nurses pass IELTS or OET, the NMC CBT (which you can take in the Philippines) and the OSCE in the UK; doctors use PLAB or an accepted postgraduate qualification; allied health professionals apply to the HCPC on paper.
- 1Pass IELTS Academic or OET at the NMC, GMC or HCPC score, or check whether the NMC qualification route applies to your transcript.
- 2Open your NMC application, name the PRC as your regulator and pay the £167 evaluation fee; the NMC then contacts the PRC.
- 3Sit the CBT at a Pearson VUE centre in the Philippines once the NMC confirms you need the Test of Competence.
- 4Accept an offer from a Home Office licensed health or care employer and apply for the Health and Care Worker visa (£324 for up to three years, no immigration health surcharge).
- 5Take the OSCE at an NMC centre in the UK, complete your ID check and join the register.
Watch out: GOV.UK says recruitment rules depend on where you live when you apply. No one may charge you a fee for finding you a UK job. The NMC's total for evaluation, tests and registration is £1,226 if you pass first time, excluding visa and travel.
Ireland
NMBI compares your Philippine nursing or midwifery programme with Irish and EU standards and may ask for a compensation measure. Most registered nurses and midwives qualify for the Critical Skills Employment Permit.
- 1Create a MyNMBI account and pay the €350 overseas recognition fee.
- 2Have your school complete NMBI's qualification form and your employers complete its employment forms.
- 3Arrange a Certificate of Current Professional Status from the PRC, issued within six months.
- 4Pass IELTS Academic or OET at NMBI's scores in one sitting, at a test centre.
- 5Complete the RCSI aptitude test in Dublin or an adaptation period in an approved hospital if required, then register (€145).
Watch out: NMBI says only you may pay its fees and you must not share your MyNMBI login with anyone, including agents. Doctors apply to the Irish Medical Council through EPIC and, unless exempt, PRES.
Germany
Nurses are recognised as Pflegefachfrau or Pflegefachmann by the authority of the federal state where you will work, and doctors apply for the Approbation. German at B2, and for doctors a C1-level professional language exam, is the longest part of the route.
- 1Start German early and reach B2 with a recognised certificate.
- 2Use the Recognition Finder to find the competent authority in your chosen state.
- 3Have your documents apostilled by the DFA where required and translated into German by a qualified translator.
- 4Apply and receive the notice (Bescheid) listing any substantial differences.
- 5Close the gaps with a knowledge test or adaptation period, often while employed on a section 16d recognition visa (about A2 German needed).
Watch out: The authority can offset differences with documented work experience gained in any country, so detailed employment certificates matter. Some states require translations even of English documents.
Netherlands
The BIG register's certificate of professional competence route is open to Filipino-trained nurses, doctors and other registered professions. You need Dutch at the level for your profession and, for nurses, doctors and dentists, the BI-test in the Netherlands.
- 1Reach Dutch B1 (nurses) or B2 or B2+ (other professions) and get an English reading certificate.
- 2Have copies certified by an authorised body; the BIG register does not accept an apostille as certification.
- 3Apply free of charge through mijn.bigregister.nl with your diploma, syllabus and declaration of competence.
- 4Sit the BI-test if your profession has one (€400 for nurses) and complete any extra training the CBGV advises.
- 5Register for €85 and work three months under supervision.
Watch out: You need a Certificate of Current Professional Status and a certificate of good conduct, each no more than three months old, at registration. The BIG register has said BI-test places for doctors are full.
United States
Filipino nurses can take the NCLEX-RN in the Philippines, and professional nurses are a Schedule A occupation for the EB-3 green card. Doctors follow the ECFMG certification, USMLE and residency route.
- 1Choose a state board and order the credentials evaluation it accepts, usually TruMerit's CES Professional Report.
- 2Have your school and the PRC send documents directly to the evaluator.
- 3Pass an English test that meets both your board and VisaScreen, unless your board does not require one.
- 4Get your Authorization to Test and sit the NCLEX-RN at a Pearson VUE centre in the Philippines.
- 5Work with a sponsoring employer on the I-140 petition and obtain your VisaScreen certificate ($740) before the visa is issued.
Watch out: Green card waiting times depend on your priority date and country of birth in the State Department's monthly Visa Bulletin. Read contracts for commitment periods and repayment terms, which vary by employer.
Documents to gather
PRC certificate of good standing, board rating and licence
Request your Certificate of Good Standing, Certification of Board Rating and Certificate of Passing through the PRC's online system (LERIS, at online.prc.gov.ph), under the Certifications tab, and keep a clear copy of your PRC professional identification card. Some destinations want the certificate sent to them directly by the PRC, and most set a short validity window, so request it only when the destination's application is ready. Renew your PRC licence before you start: an expired licence cannot be verified as current.Transcript, diploma and detailed course content from your school
Ask your college or university registrar for your transcript of records, diploma and a course description showing theory and clinical hours by subject and placement area. NMBI needs its own qualification form signed by the head of nursing or midwifery education, the HCPC needs its Course Information Form completed by your university, and TruMerit and ECFMG need documents sent directly by the school. Schools can take weeks to respond, so contact the registrar before you pay any application fee.Apostille from the DFA where the destination asks for it
The Philippines has been a party to the Apostille Convention since 14 May 2019, and the DFA issues apostilles on Philippine public documents. Germany's authorities may ask for an apostille or legalisation on foreign documents; UK regulators ask for certified copies rather than apostilles; and the Dutch BIG register does not accept an apostille as certification and wants copies certified by an authorised body instead. Ask the DFA which prior authentication your document needs before it can be apostilled (for example, school records), and apostille only what your destination actually asks for.Employment certificates and references from every hospital
Get certificates of employment on hospital letterhead with exact dates, wards or units, your position and whether you worked full time. NMBI can count post-qualification experience only if your employers complete its employment forms, the HCPC asks for referees who supervised you and can be contacted on an official work email, and the NMC recommends checking with referees and verifiers before you submit. Vague letters that give only start and end dates are the most common gap.Police clearance
Several destinations ask for a police or good conduct certificate: Germany's Recognition Finder lists a certificate of good conduct from your country of origin, no more than three months old, CORU asks for police clearance from each country where you lived for one year and one day or longer since age 18, and the BIG register asks for a certificate of good conduct no more than three months old at registration. In the Philippines this is usually the NBI clearance; ask your destination which form and how recent it must be before you apply.Civil registry documents for name changes
If your name differs between your passport, diploma, PRC licence and English test certificate, for example after marriage, get the supporting certificate from the Philippine Statistics Authority. The NMC asks for a marriage, civil partnership or divorce certificate or a deed poll, and ECFMG warns that name mismatches are a frequent cause of delay. Use your name exactly as it appears on your passport when you book NCLEX, IELTS or OET.Translations, mainly for Germany
Most Philippine professional and academic documents are issued in English, which the UK, Ireland, the USA and the Netherlands accept without translation. Germany is different: the Bezirksregierung Münster in North Rhine-Westphalia requires qualified translations of all foreign-language documents, including English ones, by a translator publicly appointed in Germany or confirmed by the German mission. Budget for this before you apply for German recognition.English test results sent directly
Ask IELTS or OET to send your results to each regulator or credentialing body directly, or give your test report number where the regulator verifies scores itself (NMBI and the NMC both do). Results are generally valid for two years, and they must still be valid when the regulator assesses or approves your application, not just when you apply. Book the test once your documents are moving, not years ahead.
Verification and good standing
- NMC (UK nurses and midwives): you name the PRC as your regulator and upload a copy of your registration certificate, and the NMC contacts the PRC directly to confirm your details. The NMC says that if your regulator cannot confirm you meet its criteria, for example because you have not completed a registration examination, it may not be able to accept your eligibility application, so pass the Philippine licensure exam and register with the PRC first.
- GMC (UK doctors): your primary medical qualification is verified by ECFMG through EPIC, which the GMC says most schools answer within 90 days. You also need a certificate of good standing from each medical regulator you have been registered or licensed with in the last five years, valid for three months from signature and still valid when the GMC approves your application.
- Irish Medical Council: EPIC reports for your degree and internship must reach the Council before you submit, and every registration body from the past five years must email a certificate of good standing directly to [email protected], dated within three months and still in date when your registration is activated.
- NMBI (Irish nurses and midwives): a Certificate of Current Professional Status issued in the last six months is needed from every jurisdiction where you registered or practised. You can tick the option in MyNMBI for your nursing council to send it to NMBI directly.
- HCPC (UK allied health): the HCPC verifies details with your education provider, regulators and referees, and since 20 November 2025 it no longer asks for more information after you submit, so check that your PRC registration and university contacts are correct before you pay the scrutiny fee.
- USA (nurses): most boards accept TruMerit's CES Professional Report, for which your school sends transcripts and the PRC sends licence validation directly to TruMerit; documents from you are not accepted as primary source. TruMerit says licence validations must carry issuing authority signatures dated within three years, and some states require nine months. Some boards, such as Texas, want verification from every country or state where you have ever held a licence.
- USA (doctors): ECFMG verifies your final diploma and transcript directly with your medical school through MyIntealth. If you have held an unsupervised licence on or after 1 January 2022 you use Pathway 1, and each licensing authority sends a certificate or letter of good standing directly to ECFMG.
- Netherlands and Germany: the BIG register asks for a Certificate of Current Professional Status from your country of training and any country where you lived or worked for three months or more, no more than three months old, and German authorities want a certificate of good standing or equivalent no more than three months old when submitted. Time your PRC request to each window.
English tests
- No destination we cover exempts you because you trained in the Philippines. The Philippines is not on the NMC's list of majority English-speaking countries (last updated January 2025), not on the HCPC's list of qualifying countries, and not among the countries NMBI, the Irish Medical Council or TruMerit's VisaScreen name for exemption.
- NMC: the qualification route is open to nurses trained in English in a country that is not majority English-speaking only if your transcript shows at least half your time was spent interacting with patients, service users, families and other professionals, with at least 75% of those interactions in English; supporting information from a UK employer (SIFE) can be added to the transcript. If you cannot show this, you need IELTS Academic (7 in listening, reading and speaking, 6.5 in writing) or OET (B in listening, reading and speaking, C+ in writing).
- Other UK and Irish scores: GMC IELTS Academic 7.0 in each skill and 7.5 overall or OET Medicine grade B in each sub-test; HCPC IELTS 7.0 overall with no element below 6.5 (8.0 and 7.5 for speech and language therapists) or profession-specific OET where offered; NMBI IELTS Academic 7.0 overall with 7.0 in three components and 6.5 in one, or OET B in three and C+ in one, from one sitting; Irish Medical Council OET Medicine B in each module or IELTS Academic 7.0 overall with 6.5 in each module.
- USA: VisaScreen for registered nurses (TruMerit table effective 12 May 2026) needs IELTS 6.5 overall with 7 in speaking, or OET 360 speaking, 340 reading, 350 writing and 320 listening, with no at-home versions. Choose one test and score that meets both your state board and VisaScreen. ECFMG accepts only OET Medicine (350 in listening, reading and speaking, 300 in writing) for doctors, with no exemptions.
- Test centres: IDP runs IELTS in 22 Philippine cities, including Manila, Quezon City, Makati, Pasig, Cebu, Davao, Baguio, Iloilo and Cagayan de Oro, on paper and on computer, and offers IELTS for UKVI. OET is also delivered in the Philippines; use OET's venue finder for current venues and dates. ECFMG allows OET@Home only for candidates with no test venue in their country.
- Online and combined results: NMBI and the Irish Medical Council do not accept IELTS online or OET at home, NMBI does not accept combined results from two sittings, and the NMC accepts two sittings of the same test within 12 months under its rules. Check your regulator's rule before you book a retake.
- Germany and the Netherlands: English is not the licensing language. German authorities usually expect a B2 general German certificate and a professional language exam pitched around C1 for doctors; Dutch BIG registration needs Dutch at B1 for nurses and B2 or B2+ for other professions, plus an English reading certificate (A2 for nurses).
Sitting exams from here
- NCLEX-RN (USA): the Philippines is on NCLEX's list of international testing locations, at Pearson VUE centres. Testing outside the US adds a $150 non-refundable international scheduling fee to the $200 registration fee, which NCLEX says rises to $350 from 1 February 2027.
- NMC CBT (UK, Part 1 of the Test of Competence): delivered by Pearson VUE, which the NMC says has test centres in most countries; use Pearson VUE's NMC test centre search to find a centre in the Philippines. The fee is £83.
- NMC OSCE (UK, Part 2): only in the UK, at the NMC's approved centres: the University of Northampton, Ulster University and Leeds Teaching Hospitals NHS Trust. Many UK employers arrange and pay for it after you arrive; check your offer.
- PLAB 1 (UK doctors): held in UK cities, Dublin, Madrid and at British Council centres overseas in countries including India, Pakistan, Bangladesh, Egypt, Nigeria, Saudi Arabia and the UAE. Check the GMC's PLAB 1 page for whether a centre in the Philippines is offered for your sitting. PLAB 2 is held only in Manchester.
- USMLE Step 1 and Step 2 CK (USA doctors): taken at Prometric test centres; FSMB adds a region fee of $210 (Step 1) or $235 (Step 2 CK) for testing outside the US and Canada. Use Prometric's locator to check centres near you. Step 3 is taken only in the US.
- Ireland: the RCSI aptitude test for nurses is held in person in Dublin, and the Irish Medical Council's PRES Level 2 is held in Dublin and Dubai in 2026. Germany's Fachsprachprüfung and Kenntnisprüfung, and the Netherlands' BI-test, are all taken in those countries.
Ethical recruitment rules
- WHO: the WHO Health Workforce Support and Safeguards List 2023 names 55 countries with the most pressing health workforce challenges. The Philippines is not on it; GOV.UK's red list, which is drawn from the WHO list, does not include the Philippines.
- UK code of practice (updated 27 March 2025): GOV.UK grades the Philippines green with a government-to-government agreement, alongside India, Malaysia and Sri Lanka, and says active recruitment is permitted from green countries. It says a new UK-Philippines memorandum of understanding signed in October 2021 formalises the relationship at government level for Filipino nurses to work in the NHS, so that they are recruited ethically, supported to reach their potential and have their wellbeing protected.
- Fees: NHS Employers' summary of the code says international health and social care staff will not be charged fees for recruitment services relating to getting a job in the UK, and GOV.UK notes that charging a work-finding fee is illegal under section 6(1) of the Employment Agencies Act 1973. Employers may choose to pay visa fees. Any repayment clause must be transparent, proportionate, time-limited and flexible, and only genuine, evidenced expenses can be reclaimed.
- Which rules apply: GOV.UK says the rules on active recruitment and direct applications depend on the country where you live when you apply, not your nationality. NHS Employers keeps an ethical recruiters list of agencies that have agreed to follow the code; being on it does not make an agency a preferred supplier.
- Department of Migrant Workers (DMW): the DMW was created by Republic Act No. 11641 (2021) to regulate the recruitment, employment and deployment of overseas Filipino workers and the operations of private recruitment agencies. Before you leave to take up a job abroad, check the DMW's current requirements for your type of hire (through a licensed agency or as a direct hire), including the documents and orientation it requires before departure, on dmw.gov.ph or at a DMW office.
- These rules govern how employers and agencies recruit and how deployment is documented; none of them stops you applying to a regulator yourself. You can open your NMC, NMBI, BIG or US board application at any time.
Questions
Usually yes. The Philippines is not on the NMC's list of majority English-speaking countries, and NMBI, the HCPC and VisaScreen do not exempt it either. The NMC's qualification route can work if your transcript shows at least half your time interacting with patients and others, 75% of it in English; otherwise you need IELTS Academic or OET.
Yes. The Philippines is on NCLEX's list of international testing locations at Pearson VUE centres. Testing outside the US adds a $150 international scheduling fee to the NCLEX registration fee, which is $200 and rises to $350 from 1 February 2027.
The CBT is delivered by Pearson VUE, which the NMC says has test centres in most countries. Use Pearson VUE's NMC test centre search to book a centre near you. The OSCE, Part 2 of the test, is only held in the UK.
Request it through the PRC's online system at online.prc.gov.ph, under Certifications, where you can also order a Certification of Board Rating and Certificate of Passing. Check whether your destination wants the PRC to send it directly, and request it close to when you need it, because the GMC, Irish Medical Council and BIG register want certificates no more than three months old.
It depends on the destination. Germany may ask for an apostille or legalisation, while UK regulators ask for certified copies and the Dutch BIG register does not accept an apostille as certification at all. Apostille only what your destination asks for, and ask the DFA what prior authentication each document needs.
No. GOV.UK grades the Philippines green with a government-to-government agreement, so active recruitment is permitted, and a UK-Philippines memorandum of understanding signed in October 2021 covers Filipino nurses working in the NHS. The Philippines is also not on the WHO Health Workforce Support and Safeguards List 2023.
No. The UK code of practice says international health and care staff must not be charged fees for recruitment services relating to getting a job in the UK. If a contract asks you to repay costs if you leave early, the clause must be transparent, proportionate, time-limited and flexible.
For the UK, in practice yes: the NMC confirms your eligibility with your regulator and says it may not accept your application if your regulator cannot confirm you meet its criteria, for example because you have not completed a registration examination. US boards and TruMerit also ask the PRC to validate your licence.
PLAB 1 is held in UK cities, Dublin, Madrid and at British Council centres in several countries, including Dubai and Riyadh. Check the GMC's PLAB 1 page for the current list of overseas centres before you book. PLAB 2 is held only in Manchester.
The UK registration steps can be completed in months, and the Health and Care Worker visa usually takes about three weeks once you apply. The US route includes a green card petition whose timing depends on your priority date and country of birth in the Visa Bulletin, so it can take much longer. Your own timeline depends on your English test, documents and employer.
The Department of Migrant Workers regulates the recruitment and deployment of overseas Filipino workers. Check its current requirements for your type of hire, through a licensed agency or as a direct hire, on dmw.gov.ph or at a DMW office before you book your flight.
It can. NMBI counts post-qualification experience if your employers complete its forms, German authorities can offset gaps in your training with documented experience, and the HCPC assesses experience against its standards of proficiency. It does not replace the NMC Test of Competence or the NCLEX.
