Work as a lab scientist in Germany: MT recognition (2026)

Biomedical analysis and the preparation of histology and cytology specimens are reserved in Germany for the MT für Laboratoriumsanalytik. This is how laboratory scientists trained abroad earn that title.

Overview

If you trained abroad as a medical laboratory scientist, biomedical scientist or clinical laboratory technologist, the German title you need is Medizinische Technologin or Medizinischer Technologe für Laboratoriumsanalytik (MT für Laboratoriumsanalytik).

It is governed by the MT-Berufe-Gesetz (MTBG) and replaced the older MTLA title on 1 January 2023. The permission to carry it comes from the authority of the state where you will work, and it then applies throughout Germany.

Why it matters in a laboratory: § 5 MTBG makes two groups of tasks in human medicine the preserve of this title. The first is running biomedical analysis processes with biological, chemical and physical methods. The second is preparing histological, cytological and other morphological specimens for a doctor's diagnosis. Easy-to-handle quantitative and qualitative tests on body fluids and excretions are carved out, so point-of-care style testing is not reserved, but bench work in clinical chemistry, haematology, microbiology or histopathology generally is.

The authority first checks your training against the German MT laboratory curriculum, which is set out in Annex 1 of the training and examination ordinance (MTAPrV). That annex spans haematology and haemostaseology, immunology, transfusion and transplantation medicine, clinical chemistry and endocrinology, molecular genetics, infection diagnostics (bacteriology, mycology, parasitology, virology, serology) and histology and cytology. A degree that was strong in chemistry and microbiology but light on tissue work or blood banking is where gaps tend to show.

Where the authority finds substantial differences and your training was outside the EU, EEA or Switzerland, § 51 MTBG lets you pick between a knowledge test (Kenntnisprüfung) on state exam content and an adaptation course of at most three years. Laboratory work in the Gulf, India, the Philippines or elsewhere can shrink those gaps under § 48. The three state profiles we checked (Berlin, North Rhine-Westphalia and Bavaria) all promise a decision within three months of a complete file and all set German at B2.

Eligibility

  • A medical laboratory qualification that your training country recognises and that lets you practise there (NRW asks for proof of that authorisation).
  • Training content the authority judges equivalent to the German MT Laboratoriumsanalytik course in Annex 1 MTAPrV, or gaps closed by a knowledge test or adaptation course.
  • Documented coverage of the core disciplines: clinical chemistry, haematology and coagulation, immunohaematology and transfusion, microbiology and infection serology, histology and cytology.
  • German at B2 for the profession (§ 1 MTBG requires the German needed to practise); in Bavaria usually tested through a specialist language examination.
  • Personal reliability, shown with a criminal record extract, and health fitness for laboratory work, shown with a medical certificate; NRW lets you send both later in the procedure.
  • Any work experience you want counted, documented in enough detail to show which analysers, methods and departments you covered.
  • For non-EU nationals, a residence title that fits your stage: a job, a place on an adaptation course or an exam date.

Step by step

  1. Map your training against Annex 1 MTAPrV

Before you contact anyone, list the hours and practical placements your course gave you in each Annex 1 discipline: clinical chemistry, haematology and haemostaseology, immunohaematology, microbiology, molecular methods, histology and cytology, and quality management. Ask your university or college for a module handbook that shows these hours. This mapping tells you early whether histology, cytology or transfusion work will be flagged.
  2. Apply to the state laboratory recognition desk

Berlin applications go to the Landesprüfungsamt für Gesundheitsberufe at LAGeSo, which also accepts them through the Berlin service portal. North Rhine-Westphalia routes every MT application to the ZAG-PuG unit of the Bezirksregierung Münster, and Bavaria to the Bayerisches Landesamt für Pflege in Amberg. Send the German CV, translated diploma, module handbook, proof you may practise in your training country and work references; the authority confirms receipt within a month.
  3. Let the training check run first

§ 44 MTBG makes the authority examine your qualification before it looks at reliability, health or German. That is why NRW asks for the police certificate and medical certificate only later: they would otherwise expire while the file is assessed. Use the waiting time for German lessons rather than for paperwork that may go stale.
  4. Read the deficit notice closely

Within three months of a complete file you receive a Bescheid. If it finds substantial differences under § 47 MTBG, it must say which Annex 1 subject areas are missing and why your laboratory experience did not offset them under § 48. Check every named gap against your references: a missed CV detail such as two years on a blood bank bench can sometimes be put right before you commit to a measure.
  5. Choose the knowledge test or the adaptation course

§ 51 MTBG gives you the choice. The knowledge test suits people with broad bench experience who can revise for an oral on biomedical analysis and quality management and handle two to four practical tasks. The adaptation course suits people with a clear gap, such as no histology training, because it lets you learn the missing techniques in a German laboratory under supervision.
  6. Sit the Kenntnisprüfung

The state must give you the chance to sit it within six months of the decision (§ 75 MTAPrV). The oral part covers competence areas I and II of Annex 1 (analysis processes, and quality, risk, process and data management) before two school examiners. The practical part is two to four tasks of up to 120 minutes each, examined by a school examiner and a practical examiner at a site the authority picks.
  7. Or complete the adaptation course in a laboratory

The authority sets the length and content of the course from your gaps (§ 89 MTAPrV). You practise under the responsibility of someone who holds the MT title, with teaching at an approved MT school and practical training at an approved site (§ 90). It ends with a final discussion; if that is not passed, the course can be extended once and then repeated once (§ 94).
  8. Show B2 German for laboratory work

Berlin and NRW accept the language certificate after the application. In Bavaria you usually prove job-related German through a specialist language examination. Laboratory German is heavy on method names, units, reporting critical values and talking to wards by phone, so practise those situations as well as general B2.
  9. Collect the title and register with employers

When equivalence or a passed measure, reliability, health and German are all confirmed, the authority issues the Erlaubnis to use the title MT für Laboratoriumsanalytik. You can also ask for a separate notice confirming equivalence (§ 45 MTBG), which some employers and visa offices find useful. From then on you may carry out the § 5 reserved tasks without supervision.

Exams

  • Kenntnisprüfung, oral part

Covers competence areas I and II of Annex 1 MTAPrV: planning, running, documenting and assessing biomedical analysis processes, and quality, risk, process and data management. It lasts 45 to 60 minutes per candidate and is taken by two school examiners at a venue the authority sets (§§ 77 to 79). A failed oral can be retaken once on request (§ 81).

  • Kenntnisprüfung, practical part

Two to four tasks drawn from the Annex 1 competence areas, chosen by the authority, each meant to last no more than 120 minutes (§ 82 MTAPrV). A school examiner and a practical examiner assess you and may question you about what you are doing (§ 84). Each failed task can be repeated once (§ 86).

  • Anpassungslehrgang with final discussion

At most three years (§ 51 MTBG), with the length and content fixed by the authority (§ 89 MTAPrV). It combines supervised practice with teaching and closes with an assessment in the form of a final discussion (Abschlussgespräch) under § 90. One extension and one repeat are allowed (§ 94).

Language

  • German, not English, is the requirement: § 1 MTBG asks for the German needed to practise, and the Berlin, NRW and Bavaria profiles set this at B2.
  • In Berlin and NRW the B2 certificate can follow the application; the training check runs without it.
  • Bavaria expects job-related B2 and says it is mostly tested through a specialist language examination.
  • No certificate is needed if German is your first language or you trained in German.
  • English-language laboratory certifications (for example ASCP) do not count as language evidence or as recognition.

Timeline

  • Receipt of your laboratory MT application confirmed within one month (Berlin, NRW and Bavaria profiles).
  • Training decision within three months of a complete file in all three states; Berlin notes it can be extended in specific cases.
  • Knowledge test: § 75 MTAPrV obliges the state to offer you a sitting within six months of the decision, so the one-month, three-month and six-month limits together put the end of the test route within about a year of a complete application, if your German is ready.
  • Adaptation course: whatever length your notice sets, up to three years; a single extension is possible if the final discussion is not passed.
  • B2 German for laboratory work: commonly the step that decides the overall date, so start it before the module handbook request.

Pay

Hospital laboratories run by municipal employers pay on the TVöD (VKA). The rows below are monthly gross Anlage A table rates valid from 1 May 2026, taken from the VKA hospitals edition. Its Entgeltordnung has not yet been renamed for the MT titles and still lists state-examined medical technical assistants in EG 7, moving to EG 8 where at least a quarter of the work is difficult. Confirm with the hospital how it maps MT für Laboratoriumsanalytik. Step 2 needs at least one year of relevant experience and step 3 usually three. Private laboratory groups, university hospitals and church employers have their own agreements or pay scales.

Employers

  • Central laboratories of hospitals, including university hospitals, which run clinical chemistry, haematology, microbiology and transfusion services.
  • Pathology and cytology institutes, where specimen preparation reserved by § 5 MTBG is the core work.
  • Large private laboratory groups and medical care centres (MVZ) for laboratory medicine, which process samples from practices and smaller hospitals and set their own pay.
  • Blood donation services and transfusion medicine departments.
  • If you take the adaptation route, you need a laboratory and an approved MT school willing to host it; ask at interview whether they have trained foreign colleagues before, and agree your job title and pay until the Erlaubnis arrives.

Visa

  • MT für Laboratoriumsanalytik is a vocational qualification in Germany, so once you hold the title the usual permit is the skilled-worker residence permit for vocational training (§ 18a AufenthG).
  • Laboratory scientists with a gap such as histology or transfusion often choose the adaptation course; the § 16d(1) residence permit for qualification measures covers up to three years in total, which matches the legal maximum of the course.
  • If a hospital or laboratory group offers you a job before recognition, the recognition partnership under § 16d(3) lets you start work and complete the procedure in Germany, provided your training lasted at least two years (or you hold a degree) and the employer commits to enabling the measure.
  • Every residence option, its conditions and the visa fee are explained on our Germany country guide.

Moving from the Gulf

  1. 1

Gulf laboratory licences, whether a DHA, DOH or MOHAP licence as medical laboratory technologist or an SCFHS laboratory classification, are not recognised as such in Germany. The authority looks at the training behind them, so the transcript from your home college or university is the key document.

  2. 2

Gulf hospital laboratory experience counts under § 48 MTBG, which disregards where the experience was gained. A reference that names the sections you rotated through (blood bank, histopathology, cytology, microbiology, special chemistry) and the analysers or methods you used is far more useful than a one-line service certificate.

  3. 3

Common gaps for Gulf-based applicants are histology and cytology preparation, when they worked only in core chemistry or haematology, and immunohaematology, when the blood bank was staffed separately. If you are still in post, ask to rotate into those sections and get it documented.

  4. 4

Quality and accreditation work done in Gulf laboratories (CAP or ISO 15189 audits, internal quality control, method validation) maps onto competence area II of Annex 1 MTAPrV, which the oral knowledge test examines. Keep evidence of it.

  5. 5

Request good-standing letters from each Gulf regulator that licensed you, and a police certificate from your home country, but time them to the stage at which your state asks; NRW, for example, asks for conduct and health evidence only after the training check.

  6. 6

German is usually new for laboratory staff from the region. Courses that cover laboratory vocabulary and phoning results to wards are worth more than general courses once you pass B1.

Questions

Medizinische Technologin or Medizinischer Technologe für Laboratoriumsanalytik, shortened to MT für Laboratoriumsanalytik. It replaced MTLA in 2023.

Only in tasks not reserved by § 5 MTBG, such as simple tests on body fluids, or under supervision during an adaptation course. Biomedical analysis and histology or cytology specimen preparation need the title, subject to the exceptions in § 6.

An oral of 45 to 60 minutes on competence areas I and II of Annex 1 MTAPrV, and two to four practical tasks of up to 120 minutes each. Each part can be retaken once.

The ordinance says the state must give you the opportunity within six months of the decision that you need it.

The authority is likely to list it as a substantial difference unless your work experience covered it. You can then sit the knowledge test or take an adaptation course that includes histology training.

Neither replaces German recognition. The experience behind them can still offset gaps if your references describe it in detail.

Berlin, NRW and Bavaria state a decision within three months of a complete file. Add the knowledge test or adaptation course and the time to reach B2.

Under the VKA TVöD, state-examined medical technical staff are in EG 7, from €3,294.98 to €4,045.24 a month from 1 May 2026, or EG 8 where at least a quarter of the work is difficult.
