Work abroad as a physician associate: UK, Ireland, EU, US
Of the five countries MedicSail covers, only the UK and the Netherlands have a registration route for physician associates trained abroad. In Ireland and Germany the role is not regulated by law, and in the USA you must graduate from an accredited US programme.
Overview
Physician associates (PAs), called physician assistants in many countries, are trained to take histories, examine patients and help diagnose and manage illness under the supervision of a doctor.
If you trained as a PA outside the UK, the short answer is that the UK and the Netherlands are the two countries here with a formal route for you. The UK route runs through the General Medical Council (GMC): a qualification assessment, the PA Registration Assessment (PARA) and then registration. The Netherlands route runs through the BIG register, where physician assistant is one of 11 registered professions.
The UK is in the middle of change, and this page sticks to what official sources confirm. The GMC began registering PAs in December 2024, and its transition period ends on 13 December 2026, after which it will be an offence to practise as a PA in the UK without GMC registration. In July 2025 the government accepted all 18 recommendations of the independent Leng review, including renaming the role 'physician assistant' and stopping PAs from treating undiagnosed patients except in clearly defined cases. The legal title change was put out to consultation in March 2026; NHS Health Careers says the consultation closed on 21 July 2026 and a government response will follow later this year. Until the law changes, 'physician associate' remains the regulated title.
Elsewhere the picture is very different. CORU, the Irish regulator for health and social care professionals, has no registration board for PAs, and the Medical Council registers doctors, so Ireland has no statutory register or recognition route for PAs. Germany teaches Physician Assistance as a university degree, but there is no federal profession law, protected title or state authorisation; PAs work on tasks delegated by doctors, and employers decide whom to hire. In the USA, the National Commission on Certification of Physician Assistants (NCCPA) only lets graduates of programmes accredited by ARC-PA sit its exam, so a foreign PA qualification does not give you a route to licensure.
Demand varies as much as regulation. The US Bureau of Labor Statistics projects employment of physician assistants to grow 21 percent from 2025 to 2035, but that market is closed to foreign-trained PAs without US education. In the UK, NHS workforce plans for the role are being reset after the Leng review, and the GMC itself strongly recommends that international applicants check their prospects of finding a PA job before paying for assessment.
This page is for PAs trained outside these countries, including those working in the Gulf, and for doctors and other clinicians wondering whether a PA route is open to them. A medical degree is not a PA qualification: doctors follow the medical route in each country, and the GMC assesses only qualifications that allow the holder to practise as a PA or a comparable title.
Qualifications
Minimum
For the UK, a PA (or physician assistant or comparable) qualification that lets you practise in the country where it was awarded, from an accredited institution, lasting at least two academic years with about 3,150 hours of education and training. You must also have been accepted for registration as a PA where the profession is regulated by law, unless your qualification is from Ireland, an EFTA country or Switzerland.
Preferred
A programme grounded in scientific, evidence-based medicine with in-person clinical experience of at least 1,400 hours or 40% of course hours, covering general practice, acute and emergency medicine, general medical specialties, paediatrics, surgery, gynaecology and mental health, as the GMC's acceptable overseas qualification criteria require. For the Netherlands, a qualification the BIG register's committee finds comparable to the Dutch physician assistant master's.
Experience
The GMC sets no minimum period of work experience, but you must give five years of work history and an employer reference covering at least your most recent three months of healthcare work. For English, two years of continuous practice in a country on the GMC's English-speaking list can count instead of a test.
Country by country
United Kingdom: GMC
Physician associate (GMC register). The government has consulted on changing this to physician assistant in law.
- Requirements
- Have your qualification assessed against the GMC's nine acceptable overseas qualification criteria, pass both parts of the PARA, meet the English requirement and start primary source verification with ECFMG. Then apply with your registration history, work history, fitness to practise and indemnity declarations, and an identity check.
- Exams
- The PARA, run by the Royal College of Physicians for the GMC: a 200-question single best answer knowledge-based assessment taken online, and a 14-station OSCE at the Spine in Liverpool. You have up to four attempts at each part.
- English
- Academic IELTS with at least 7.0 in each area and 7.5 overall, or OET Medicine with grade B in each area, in one sitting and less than two years old. Two years of recent practice in a listed English-speaking country may count instead.
- Timeline
- The GMC aims to make an initial assessment of a registration application within 10 working days. The PARA runs on fixed dates with results about eight weeks after each sitting, and ECFMG says most institutions respond within 90 days.
- Good to know
- From 1 April 2026: £528 qualification assessment (non-refundable), £337 registration and £337 a year. The PARA costs £325 (KBA) plus £641 (OSCE) for the 2026/27 sittings.
Ireland: Not regulated
Physician associate (no protected title or statutory register)
- Requirements
- There is no state registration for PAs. CORU's registration boards cover other health and social care professions, and the Medical Council registers doctors. Whether you can work as a PA depends on the employer.
- Exams
- None. There is no national PA exam or recognition procedure for internationally trained PAs.
- English
- Set by the employer. RCSI, which runs an MSc in Physician Associate Studies, asks non-native speakers for Academic IELTS 7.0 with no band below 6.5 for entry to the course.
- Timeline
- No regulatory timeline applies, because there is no registration step.
- Good to know
- The HSE consolidated pay scales (1 June 2026) list no physician associate grade. Check how any post is graded before you move.
Germany: Not regulated
Physician Assistant (university degree, not a state-regulated profession)
- Requirements
- Germany has no federal profession law, protected title or state authorisation for physician assistants, so there is no recognition procedure that grants a licence. PAs work on tasks delegated by doctors, under their responsibility. Employers decide whether your qualification is suitable.
- Exams
- None set by the state. German universities award bachelor's and master's degrees in Physician Assistance.
- Language
- No statutory language rule applies, but in practice you need German for clinical work. Ask the employer what level it expects.
- Timeline
- No regulatory timeline applies. Your work visa will depend on the job and your qualification.
- Good to know
- We could not find an official national recognition procedure for PAs. The professional association is the Deutsche Gesellschaft für Physician Assistants (DGPA).
Netherlands: BIG
Physician assistant (BIG register)
- Requirements
- With a diploma from outside Europe, apply for a Certificate of Professional Competence through mijn.bigregister.nl. The Committee for Foreign Healthcare Graduates (CBGV) compares your education, diploma and work experience with the Dutch standard and may require additional training. You then apply for BIG registration.
- Exams
- No BI-test for physician assistants; the BIG register sets it only for doctors, dentists and nurses. The committee may invite you to an interview, or require an adaptation internship or practical skills test.
- Language
- Dutch at the level the BIG register sets for your profession, through the State Examination of Dutch as a Second Language (NT2), plus English reading skills. Certificates must be less than two years old.
- Timeline
- The BIG register says six to 18 months on average. For professions without a BI-test you receive an intended decision within six weeks of your file reaching the committee. Additional training can take three months to two years.
- Good to know
- The certificate procedure is free; BIG registration costs €85. You must also prove you have no foreign restrictions or criminal convictions.
United States: NCCPA and state boards
Physician assistant (PA-C after NCCPA certification; licensed by each state)
- Requirements
- NCCPA says you must graduate from an entry-level PA programme accredited by ARC-PA to be eligible for the PANCE, and all states rely on NCCPA certification for licensure. A foreign PA qualification does not make you eligible, so there is no route for internationally trained PAs other than completing an accredited programme.
- Exams
- PANCE: five blocks of 60 questions, five hours of exam time, at Pearson VUE centres throughout the year. Graduates have six years and six attempts.
- English
- Set by the PA programme you would need to complete, not by NCCPA.
- Timeline
- You can take the PANCE once in any 90-day period and three times in a calendar year.
- Good to know
- The PANCE application fee is $550. Doctors trained abroad follow the ECFMG route to become physicians, not the PA route.
The route, step by step
1. Check that your country has a route
Only the UK and the Netherlands register foreign-trained PAs. Ireland and Germany do not regulate the role, and the USA requires an ARC-PA accredited programme. Decide where you can realistically work before you spend money on tests or assessments.2. Check your qualification against the criteria
For the UK, read the GMC's nine acceptable overseas qualification criteria: course length, about 3,150 hours, at least 1,400 hours or 40% in-person clinical experience, and rotations in seven named areas. The most common problem is criterion 2: you must have been registered as a PA where the profession is regulated by law. The GMC fee is not refunded if you do not meet a criterion.3. Ask your university for the evidence early
The GMC needs a letter from your university confirming course length in years and clock hours, any online study, in-person clinical hours, who oversaw them and which specialties they covered, plus transcripts. The BIG register also needs certified documents and course overviews. Universities can be slow to respond.4. Prove your English or Dutch
The GMC asks for Academic IELTS 7.0 in each area and 7.5 overall, or OET Medicine at grade B in each area. The Netherlands needs Dutch through the NT2 State Examination and English reading skills. Results usually expire after two years, so time your test with the rest of the route.5. Pass the assessment
In the UK, once the GMC accepts your qualification it tells the Royal College of Physicians that you can sit the PARA: an online knowledge test and an OSCE in Liverpool. In the Netherlands the committee may interview you or require extra training. Prepare for the specific format and dates.6. Verify your qualification and good standing
The GMC requires primary source verification of your PA qualification by ECFMG, and a certificate of current professional status from any regulator you have been registered with in the last five years. The BIG register asks for proof that you have no foreign restrictions or convictions. Request these early.7. Apply for registration
Apply to the GMC within two years of passing the PARA, or to the BIG register after you receive your certificate. The GMC keeps an application open for three months, so have your references and documents ready before you start.8. Find a job and sort out your visa
Check with employers before you begin, as the GMC itself advises. In the UK, the Health and Care Worker visa needs an eligible occupation code, a licensed health or care sponsor and the right salary. Check the official immigration rules for the country before you resign.
Exam preparation
The PARA is the main exam an internationally trained PA sits in these countries. It is developed and delivered by the Royal College of Physicians for the GMC, and has two parts: a 200-question single best answer knowledge-based assessment (KBA) taken online through Orzone, and an OSCE with 14 clinical stations at the Spine in Liverpool. From 13 December 2026 both parts assess the 2023 Physician Associate Curriculum only. In the USA the PANCE is open only to graduates of ARC-PA accredited programmes, and the Netherlands has no BI-test for physician assistants.
Tips
- Study from the 2023 Physician Associate Curriculum, as the RCP says PARA sittings after the GMC transition period ends on 13 December 2026 assess that curriculum only.
- Download the RCP's PARA blueprints, regulations and OSCE sample scenarios and practise with them before booking.
- Plan both parts together: you must pass the second part within 18 months of passing the first, or retake the first.
- Know the attempt limit: four attempts at the KBA and four at the OSCE, with no further attempts after that.
- Set up and test your computer, webcam and internet early for the online KBA, and keep a backup connection such as a phone hotspot.
- For the OSCE, practise timed stations: two minutes' reading and eight minutes in each station, and dress as you would for a UK hospital, bare below the elbow.
- Learn UK practice: GMC professional standards for PAs, consent, safeguarding and when to escalate to a supervising doctor.
- Book in the application window: payment must be made in full by card when you apply, and late payments are not accepted.

Pay by country
Only the UK has a national public pay scale used for PAs. NHS Health Careers says PAs typically start on Agenda for Change Band 7, with internships at Band 6. Ireland's HSE consolidated scales list no PA grade, and we could not confirm figures for Germany or the Netherlands from an official source, so we describe how pay is set.
UK£49,387 to £56,515 (Band 7), 2026/27NHS Agenda for Change in England from 1 April 2026. Internships at Band 6: £39,959 to £48,117.

Career growth
PA careers in the UK are being reshaped by the Leng review, which the government accepted in full. Official sources confirm the direction: a renamed role, permanent faculties to set standards, and clear team structures led by a senior clinician. Specific career grades beyond that are still being worked out, so treat any promise of rapid progression with care.
Specialty experience
PAs work in general practice, acute medicine, surgery and other specialties as part of a medical team. Experience in one area can lead to more responsibility within that team, always under the supervision arrangements your employer sets.Education and supervision
Experienced PAs help teach PA students on placement and in universities. Academic posts usually need a master's degree or higher.Professional leadership
The government said permanent faculties will be set up to give professional leadership and set standards for physician assistants. These will create roles in standards, education and representation.Management and service roles
Some PAs move into service improvement, clinical governance or management posts, where pay and grading depend on the employer and the job.Retraining as a doctor or another profession
Some PAs go on to study medicine or another regulated profession. This is a new qualification, not a conversion, and follows that profession's own route.
Common mistakes
- 1
Paying for a GMC qualification assessment without meeting every criterion. The GMC says it is unlikely to accept your qualification if you cannot show all nine, the fee is non-refundable and you cannot appeal.
- 2
Missing criterion 2. Your qualification must be from a country where PAs are regulated by law and accepted for registration there, or you must have held PA registration in another country where the profession is regulated by law.
- 3
Not having enough in-person clinical hours, or missing one of the seven required areas, such as mental health or gynaecology.
- 4
Assuming a medical degree counts. The GMC assesses PA qualifications only; doctors use the medical route, including PLAB.
- 5
Taking the wrong English test. The GMC accepts only Academic IELTS at 7.0 in each area and 7.5 overall, or OET Medicine, not OET Nursing.
- 6
Letting the 18-month or two-year windows run out. Pass the second PARA part within 18 months of the first, and apply to the GMC within two years of passing the PARA.
- 7
Moving to Ireland or Germany expecting a licence. Neither country regulates PAs, so there is no registration to apply for and jobs depend entirely on employers.
- 8
Ignoring the job market. The GMC strongly recommends checking your chances of PA employment in the UK before you start, given the cost and time involved.

Questions
Yes, if you register with the GMC. You must have your qualification assessed against the GMC's criteria, pass both parts of the PARA, meet the English requirement and have your qualification verified by ECFMG before the GMC grants registration.
The government accepted the Leng review's recommendation to rename the role physician assistant in July 2025, and consulted on changing the title in law from March 2026. Until the law changes, physician associate is still the title the GMC registers.
The PA Registration Assessment, delivered by the Royal College of Physicians for the GMC. It has a 200-question single best answer test taken online and a 14-station OSCE at the Spine in Liverpool, and you must pass both before you can apply to register.
From 1 April 2026, £528 for the qualification assessment, £337 to register and £337 a year after that. The PARA is paid to the RCP: £325 for the KBA and £641 for the OSCE for the 2026/27 sittings.
Academic IELTS with at least 7.0 in each area and 7.5 overall, or OET Medicine with grade B in each area, both in one sitting and less than two years old. Two years' recent practice in a listed English-speaking country may count instead.
The GMC says that from December 2026, when its two-year transition period ends, it will be an offence to practise as a PA in the UK without GMC registration.
No. CORU has no registration board for PAs and the Medical Council registers doctors, so there is no statutory register or recognition route. Employment depends on the employer.
Germany offers Physician Assistance degrees, but the role is not state-regulated and has no protected title or licence. PAs work on tasks delegated by doctors, and employers decide whom to hire.
Yes. The BIG register lists physician assistant among its 11 professions. With a diploma from outside Europe you apply for a Certificate of Professional Competence and then BIG registration, which costs €85.
Not with a foreign PA qualification. NCCPA says you must graduate from an ARC-PA accredited entry-level PA programme to sit the PANCE, and states rely on NCCPA certification for licensure.
The GMC assesses qualifications that allow you to practise as a PA or a comparable title. A medical degree is not a PA qualification, so doctors trained abroad usually follow the medical route, such as PLAB.
Only if it is PA registration in a country where the profession is regulated by law, which the GMC decides case by case. You will also need a certificate of current professional status from each regulator you were registered with in the last five years.
