Occupational therapist in Germany: Ergotherapeut (2026)
Ergotherapeut is a protected title in Germany. Here is how occupational therapists trained outside the EU get permission to use it.
Overview
Occupational therapy in Germany is called Ergotherapie, and only people with state permission may call themselves Ergotherapeutin or Ergotherapeut.
The rules sit in the Occupational Therapists Act (Ergotherapeutengesetz, ErgThG) and the training and examination ordinance for the profession (ErgThAPrV). The permission is issued by one federal state but lets you use the title anywhere in the country.
The German benchmark is a three-year vocational course with a state exam: at least 2,700 hours of classroom and practical teaching plus 1,700 hours of supervised placements. Those placements are split across three fixed fields, each with at least 400 hours: psychosocial work (psychiatry and psychosomatics), motor-functional and neurological work, and work therapy (Arbeitstherapie), with 500 more hours spread between them. When an authority reviews a foreign OT qualification, it is largely this placement profile it is comparing you against.
Where the comparison shows substantial differences that your working experience does not cover, § 2 ErgThG lets you pick between a knowledge test based on the state exam and an adaptation course of at most three years that ends in an examination. The OT knowledge test is unusual in one respect: its practical part is built from the placement fields where your gaps were found, and you treat between one and three real patients in front of two examiners.
This page is written for occupational therapists who qualified abroad, including those now practising in hospitals, rehabilitation centres, special-needs schools and private clinics in the Gulf. It covers the state procedure, the exam, German, the fees that apply to OT recognition, pay under the public-sector agreement, and what to gather before leaving a Gulf post.
Recognition matters beyond the hospital too. Outpatient Ergotherapie in Germany is mostly paid by statutory health insurance, and § 124 SGB V only admits providers whose staff hold the training and the permission to use the professional title. Without the title, independent practice and most practice jobs are closed to you.
Eligibility
- A finished occupational therapy qualification that gives access to the profession in the country where you trained (diploma or bachelor's degree).
- Training the authority judges equivalent to the German three-year course, or a passed OT knowledge test, or a completed adaptation course with its final exam.
- Placement evidence across the German fields: psychiatry or psychosomatics, motor-functional or neurological work, and work-related therapy. Missing or undocumented hours in any one of these are the most likely gap.
- German at level B2 of the CEFR, as required in the Recognition Finder profiles for OT; Bavaria normally tests this with its own specialist language exam.
- Proof that you are reliable (a police certificate) and medically fit for the job, as § 2(1) ErgThG requires; the state lists the exact certificates and their maximum age.
- Proof that you were entitled to practise as an occupational therapist where you trained, for example a licence or registration certificate.
- If you are not an EU, EEA or Swiss citizen: a residence title, usually linked to an OT job offer or to a place on a compensation measure.
Your regulator
Step by step
Map your placements against Anlage 1 of the ErgThAPrV
Before you contact anyone, list your clinical placements by hours under the three German fields: psychosocial, motor-functional/neurological and work therapy. Many OT degrees abroad are heavy on paediatrics and physical rehabilitation and light on psychiatry or vocational rehabilitation, and the authority will see that immediately. Knowing your likely gap now tells you which exam area or adaptation content to expect.Request a module-level transcript from your OT school
Ask your university or college for a transcript that shows each module with its hours, and a placement record that names the setting, client group and supervised hours of each rotation. A degree certificate alone is not enough for an equivalence check. Schools can take weeks to issue these, so start while you are still working.Apply to the state office for health professions
Send the application to the office of the state where you want to work: for example LAGeSo's Landesprüfungsamt für Gesundheitsberufe in Berlin, the ZAG-PuG at the Bezirksregierung Münster for North Rhine-Westphalia, or the Bayerisches Landesamt für Pflege in Amberg for Bavaria. Include your CV in German, certificates and transcripts with German translations, experience letters and proof of your right to practise abroad. The office acknowledges receipt within a month and lists anything missing.Read the deficit notice carefully
For OT the Recognition Finder gives a maximum of four months from a complete file to the decision. If the training is not equivalent, the notice names the substantial differences, normally in terms of the Anlage 1 fields, and whether your work experience has reduced them. Check that your Gulf or home-country experience letters were taken into account before you accept it.Choose the knowledge test or the adaptation course
§ 2 ErgThG gives you the choice. The knowledge test is quicker if you can prepare for an oral exam in German on law, pathology and OT foundations and for practical patient sessions. The adaptation course suits people who want supervised clinical time in a German setting first; the authority sets its length and content, up to three years.Sit the OT knowledge test
The oral part lasts 15 to 60 minutes before two examiners. In the practical part you treat one to three patients from the fields the authority picked from your notice: you take a history and findings, present and justify your treatment plan, and treat, with up to 120 minutes per field. You may repeat the oral part and each failed field once.Reach B2 and, in Bavaria, the specialist language exam
German at B2 is a condition of the title. NRW says you do not need the certificate at the time of application, so you can apply while still learning. In Bavaria the Recognition Finder says proof is usually the authority's specialist language exam, which costs €350.Collect the permission and start work as an Ergotherapeut
When equivalence, German, the police certificate and the medical certificate are all in place, the state issues the Erlaubnis to use the title. With it you can work in hospitals and rehabilitation and be counted as qualified staff in a practice that bills statutory health insurance under § 124 SGB V.
Exams
Kenntnisprüfung, oral part
Under § 16b ErgThAPrV it lasts at least 15 and at most 60 minutes and is taken by two examiners. It covers professional and legal matters (Berufs-, Gesetzes- und Staatsbürgerkunde); specialised pathology including diagnostic, therapeutic, preventive and rehabilitative measures and psychosocial aspects; and the foundations of occupational therapy.
Kenntnisprüfung, practical part
Applied through § 16a(3): you work with at least one and at most three patients from the Anlage 1 Part B fields that the authority selects according to your differences, carrying out an assessment, presenting the treatment plan and treating. Each field takes at most 120 minutes, with two examiners. The test should be offered at least twice a year; the oral part and each failed field can be repeated once.
Anpassungslehrgang (adaptation course)
Theory and practice at suitable institutions, with duration and content set by the authority so that the identified differences are closed, up to three years under § 2 ErgThG. It ends with an examination on its content.
Language
- The title requires the German needed for the job (§ 2(1) ErgThG); the Recognition Finder profiles for OT in Berlin, NRW and Bavaria set this at B2 of the CEFR.
- NRW (Bezirksregierung Münster) says you do not have to enclose the language certificate when you apply.
- Bavaria asks for job-related German at B2 and says this is usually shown by passing its specialist language exam, with a fee of €350.
- No certificate is needed if German is your first language or you completed your OT training in German.
- There is no English-language route: therapy sessions, reports to prescribing doctors and the knowledge test are all in German.

Timeline
- Transcripts and placement records from your OT school: allow several weeks, and longer if hours have to be reconstructed from old rotation logs.
- Acknowledgement from the state office within one month; the Recognition Finder gives a maximum of four months for the OT decision once the file is complete, with extensions possible in specific cases.
- OT knowledge test: § 16b ErgThAPrV says it should be offered at least twice a year, so you may wait for a date. Each practical field takes up to 120 minutes, so the practical part can run over more than one day.
- Adaptation course: up to three years under § 2 ErgThG; the notice sets the actual length, and many are far shorter where only one placement field is missing.
- German to B2 runs alongside all of this and is often the longest part for therapists starting from zero.
Pay
Occupational therapists in municipal hospitals are paid under the TVöD (VKA). The figures below are monthly gross table rates from Anlage A, valid from 1 May 2026, as printed in the VKA hospitals edition. An Ergotherapeut with state permission is grouped in EG 7, rising to EG 8 where at least a quarter of the work is difficult; someone doing OT work without the permission is placed in EG 5. Your starting step depends on relevant prior experience, and steps rise with time in the group. Outpatient practices, private clinics and church employers pay on their own terms.

Employers
- Psychiatric and psychosomatic hospitals, which need OT for one of the three German training fields and employ many therapists.
- Neurological and orthopaedic rehabilitation clinics, early rehabilitation units and stroke services.
- Outpatient Ergotherapie practices, which bill statutory health insurance and need staff who hold the title under § 124 SGB V.
- Vocational rehabilitation and work-therapy settings, and services for children and adults with disabilities.
- Municipal hospitals follow the TVöD; university, church and private hospitals and practices set pay through their own agreements or contracts, so compare offers against the table below.
Visa
- Recognition partnership (§ 16d(3) AufenthG) can suit OTs well: an outpatient practice, rehabilitation clinic or hospital hires you, you agree in writing to apply for recognition, and the employer must enable the adaptation course or test. The first permit lasts up to one year, extendable to three, and you cannot use the title Ergotherapeut until the permission is issued.
- If your notice requires an adaptation course, its length matters: a § 16d(1) permit runs for up to 24 months plus at most 12, three years in total, so an OT course set close to the three-year maximum leaves no room for delays.
- Once recognised, OT is a vocational-training profession in Germany, so the usual long-term permit is the skilled worker route for vocational qualifications (§ 18a AufenthG).
- Full visa options, entry conditions and the visa fee are set out once, for all professions, on the Germany country guide on this site.
Moving from the Gulf
- 1
Your DHA, DOH, MOHAP, SCFHS, QCHP, NHRA or OMSB occupational therapy licence is not converted. The German state compares the course where you qualified, and § 2 ErgThG then lets real professional practice anywhere, including the Gulf, close the gaps it finds.
- 2
Gulf OT posts are often in paediatrics, special-needs schools, hand therapy and physical rehabilitation. Those count toward the motor-functional field, but they rarely fill a psychiatric or work-therapy gap, so ask any mental health or vocational rehabilitation service you worked in for a separate letter.
- 3
Experience letters should state the client groups, settings and weekly hours, and the assessments and interventions you used (for example ADL training, splinting, sensory integration). The authority can only offset differences it can match to Anlage 1.
- 4
A good-standing letter from each Gulf regulator you held an OT licence with helps show you were entitled to practise; ask the German office in advance which police certificates it wants for your country of residence.
- 5
Recognition partnership interviews and the knowledge test are in German. Gulf therapists usually work in English and Arabic, so start German before you resign, ideally while you gather your documents.
