Work as a physiotherapist in Germany: recognition (2026)
Physiotherapeut is a protected title in Germany. Here is how physiotherapists trained outside the EU get permission to use it, from the application to TVöD pay.
Overview
To treat patients as a physiotherapist in Germany you need the state permission to use the title Physiotherapeutin or Physiotherapeut, granted under the Masseur- und Physiotherapeutengesetz (MPhG).
The same act also protects a second, separate title, Masseurin und medizinische Bademeisterin or Masseur und medizinischer Bademeister, so make sure your application names the physiotherapy title. The permission is issued by one federal state and you can then use it anywhere in Germany.
German physiotherapists usually qualify at a state-recognised Berufsfachschule and pass a state exam, whereas most physiotherapists from outside Europe hold a bachelor's degree. The authority does not rank one above the other. It sets your transcript against the subjects and hours of the German curriculum in the PhysTh-APrV, and the gaps it most often finds are in the physical therapies the German exam tests in depth: massage, electrotherapy, hydrotherapy and related treatments.
Where gaps remain after your work experience is taken into account, § 2 MPhG gives you the right to pick either the Kenntnisprüfung or an Anpassungslehrgang lasting no more than three years. The physiotherapy Kenntnisprüfung is unusual because its practical part is a patient exam: you assess and treat between one and seven real patients in the specialties your notice names, within about two weeks.
German is part of the permission, not an extra. The state profiles set B2, and Bavaria, where the Bayerisches Landesamt für Pflege has handled health-profession recognition since 1 July 2025, runs its own 60-minute specialist language exam for physiotherapists. This page is for physiotherapists trained outside the EU, EEA and Switzerland, including those now working in Gulf hospitals and rehabilitation centres. In municipal hospitals recognised physiotherapists are paid on TVöD EG 7 or EG 8, while outpatient practices, which employ many German physiotherapists, agree pay individually.
Eligibility
- A completed physiotherapy qualification (diploma or bachelor's) that the state where you trained recognises as giving access to the profession there.
- Proof that you were licensed to practise physiotherapy in that country; NRW lists proof of authorisation to work in the country of training among its documents.
- Training the authority judges equivalent to German training under the MPhG and the PhysTh-APrV, or a passed Kenntnisprüfung, or a completed Anpassungslehrgang.
- Subject-by-subject hours for theory and supervised clinical practice, so the authority can compare them with the German curriculum, including massage, electrotherapy and hydrotherapy.
- German at B2 of the CEFR, which § 2 MPhG requires for the permission; in Bavaria shown through the LfP specialist language exam unless an exemption applies.
- Personal reliability (Zuverlässigkeit), shown through police certificates, and health fitness for the job, shown through a medical certificate, as § 2(1) MPhG sets out.
- References from physiotherapy employers that set out your caseload, so the authority can use your experience to close gaps.
- If you need a residence title: a German employer, school or clinic willing to host your adaptation course or support your exam preparation.
Your regulator
Step by step
Apply for the physiotherapy title, not the masseur title
The MPhG covers two professions. Use the Recognition Finder entry for Physiotherapeut and the state where you plan to work: LAGeSo in Berlin, the Zentrale Anerkennungsstelle (ZAG-PuG) at the Bezirksregierung Münster for NRW, and the Bayerisches Landesamt für Pflege in Amberg for Bavaria. Berlin accepts online, postal and in-person applications.Build a curriculum file that maps to the PhysTh-APrV
Physiotherapy files are judged on hours per subject, so ask your university or college for a detailed transcript and syllabus listing theory and clinical practice separately. Show where you covered massage, electrotherapy, hydrotherapy and thermotherapy, because these are the subjects German examiners test and the most frequent source of gaps. Add a German CV, your physiotherapy licence and translations by sworn or authorised translators; Bavaria accepts English diplomas without translation.Add your clinical experience
§ 2 MPhG lets the authority set off differences against what you learned by actually and lawfully practising physiotherapy, full or part time, or through lifelong learning. Letters that list the specialties you treated (orthopaedics, neurology, internal medicine, paediatrics) and any certified courses, such as manual lymph drainage or manual therapy, give the authority something concrete to weigh. Without them, your experience cannot reduce the gaps.Wait for the equivalence notice
Berlin, NRW and Bavaria all give four months from a complete file as the limit for a physiotherapy decision. The notice either confirms equivalence or lists the substantial differences and the specialties in which you must be tested. Read it closely: the specialties named there decide which patients you will see in a Kenntnisprüfung.Choose between the patient exam and the adaptation course
The MPhG gives you the choice. The Kenntnisprüfung suits physiotherapists with strong hands-on experience who can show German-style assessment and documentation; the Anpassungslehrgang suits those whose gaps are in whole treatment areas such as hydrotherapy or electrotherapy. In NRW the Bezirksregierung Münster publishes a list of schools offering adaptation measures for Physiotherapeut:in, and you register for the exam on its form.Prepare for German physiotherapy documentation
In the practical part you take findings (Befunderhebung), evaluate and document them, write a therapy plan with goals and treat, all in German, and examiners may question you on what you are doing. Practise writing findings and treatment plans in German before the exam, because many candidates who treat well lose marks on documentation.Pass the language requirement
The Recognition Finder profiles set B2. Berlin lets you send the certificate after you apply; NRW lists a B2 certificate among the documents. In Bavaria the LfP specialist language exam for health professions (two 20-minute dialogues and a 20-minute written task, €350) is the usual proof, and the LfP asks you to register only after the recognition office says you need it.Collect your permission and start work
With equivalence or a passed measure, plus your police and medical certificates and German, you receive the Erlaubnis zum Führen der Berufsbezeichnung Physiotherapeutin or Physiotherapeut. In NRW the ZAG-PuG decides equivalence and the permission itself is then issued by the local health authority or the Bezirksregierung. The permission is valid in every German state, so you can then work under the title in any of them.
Exams
Kenntnisprüfung, oral part
Under § 21b PhysTh-APrV it lasts at least 15 and at most 60 minutes, before two specialist examiners, with the chair of the examination board present. It covers professional and legal regulations; physiotherapy assessment and examination techniques; massage, electrotherapy and hydrotherapy; and anatomy, physiology and pathology.
Read the guideKenntnisprüfung, practical part (patient exam)
Applied from § 21a(3) PhysTh-APrV: on at least one and at most seven patients from the specialties in Annex 1 Part B that the authority chooses based on your gaps. You assess, evaluate and document findings, draw up a therapy plan with goals and carry out suitable treatment techniques. It should be finished within two weeks, and both examiners must rate it passed.
Read the guideRetakes and dates
The oral part and each failed specialty of the practical part can be retaken once. The ordinance says the Kenntnisprüfung must be offered at least twice a year, so ask your state for dates as soon as your notice arrives.
Anpassungslehrgang (adaptation course)
Up to three years under § 2 MPhG, with length and content set by the authority so that the course aim can be met. It ends with a final discussion led by a specialist examiner and your supervising instructor (§ 21b PhysTh-APrV). If that is not passed, the course can be extended once, and if still not passed it can be repeated once.
Language
- § 2(1) MPhG makes German knowledge needed for the work a condition of the physiotherapy permission.
- The Recognition Finder profiles for Berlin, NRW and Bavaria set B2 of the CEFR; no certificate is needed if German is your first language or you trained in German.
- Berlin does not require the certificate with the application but you must provide it before the permission is issued; NRW lists a B2 certificate among the documents.
- Bavaria asks for job-related German at B2 and runs its own specialist language exam in Amberg: a physiotherapist-patient dialogue, a physiotherapist-colleague dialogue and a written task, 60 minutes in total, with 60% needed in both the oral and written parts.
- English test scores (IELTS, OET, TOEFL) do not count for the German permission.
- The exam itself is in German, including the written findings and therapy plan, so aim beyond B2 in physiotherapy vocabulary.

Timeline
- Receipt of your physiotherapy application is confirmed within one month in Berlin, NRW and Bavaria; the four-month decision period starts only when the file is complete.
- Equivalence notice: within four months of a complete file, according to all three state profiles and the LfP, extendable in specific cases.
- Patient exam: offered at least twice a year under § 21b PhysTh-APrV, and the practical part itself should be completed within two weeks. Allow for months between your notice and an exam slot.
- Adaptation course: whatever length the authority sets, up to three years under § 2 MPhG.
- German: reaching B2, and passing the Bavarian specialist exam where it applies, often takes longer than the recognition procedure itself.
Pay
Physiotherapists in municipal hospitals are paid on the TVöD for VKA employers. The figures below are monthly gross table rates from Anlage A valid from 1 May 2026, as printed in the VKA hospitals edition. The Entgeltordnung puts physiotherapists with state recognition in EG 7, rising to EG 8 where at least a quarter of the work is difficult; people doing physiotherapy work without the title are in EG 5. Your step depends on relevant experience: none puts you on step 1, one year on step 2 and three years usually on step 3. Outpatient practices, rehabilitation clinics, church-run and private hospitals use other agreements or agree pay individually.

Employers
- Outpatient physiotherapy practices (Praxen), which agree pay individually; compare offers with the TVöD figures above.
- Rehabilitation clinics for orthopaedic, neurological and cardiac patients.
- Acute hospitals, including intensive care and early mobilisation after surgery, where municipal employers pay on the TVöD.
- Care homes, sports clubs and occupational health services.
- Physiotherapy schools and practices that host adaptation courses; ask employers whether they have supported a foreign-trained physiotherapist through one before, and agree your job title and pay for the period before you hold the permission.
Visa
- Physiotherapy is vocational training in Germany, so once your permission is granted your work permit route is normally the skilled-worker permit for vocational qualifications (§ 18a AufenthG), not the academic route, even if you hold a bachelor's degree.
- If your notice requires an adaptation course, the § 16d(1) residence permit for recognition measures can cover it, but it is capped at three years in total. Ask the authority to set the course length in the notice, because the visa officer will look for it.
- The recognition partnership (§ 16d(3)) fits physiotherapists hired by a practice or clinic before recognition: you start work while you complete the procedure, provided your employer agrees in writing to support the patient exam or adaptation course.
- The full visa options, conditions and fees are on the Germany country guide on this site.
Moving from the Gulf
- 1
A physiotherapy licence from DHA, DOH, MOHAP, SCFHS, QCHP, NHRA or OMSB is not transferred. The authority looks at the physiotherapy course you completed, so the transcript from your university matters more than your Gulf licence.
- 2
Gulf hospital work counts as lawful practice of physiotherapy under § 2 MPhG and can close gaps. Ask for letters that list the departments you covered (orthopaedics, neurology, ICU, paediatrics, sports rehab) and the modalities you used, including electrotherapy and hydrotherapy if your unit had them.
- 3
Many Gulf physiotherapy posts are outpatient and manual-therapy heavy, with little hydrotherapy or massage. If your letters cannot show those, expect them to appear as differences and consider the adaptation course rather than the patient exam.
- 4
Copies of short-course certificates often earned in the Gulf (manual therapy, lymph drainage, dry needling, sports taping) help only if they list hours and content; bring the course syllabus.
- 5
Your Gulf regulator can issue a certificate of good standing for your licence; check with the German authority whether it also needs a police certificate from the Gulf country where you live.
- 6
Start German while still in post: physiotherapy treatment in Germany is talked through with the patient, and the patient exam and Bavaria's dialogue-based language test both assume fluent clinical German.

Questions
It can be recognised. German training is usually vocational, but the authority compares content and practical hours, not the level of the degree, and lists any differences.
An oral part of 15 to 60 minutes on law, assessment, physical therapies and anatomy, physiology and pathology, and a practical patient exam on one to seven patients. Each failed part can be retaken once.
B2 in the Berlin, NRW and Bavaria profiles. Bavaria's LfP runs a 60-minute specialist language exam costing €350.
Yes. § 2 MPhG gives you the right to choose between the Kenntnisprüfung and an adaptation course of up to three years.
The authority decides within four months of a complete file, and may extend this. A compensation measure and German study add to that.
In municipal hospitals EG 7 runs from €3,294.98 to €4,045.24 a month from 1 May 2026. Private practices set their own pay.
Not under the protected title. Under the recognition partnership or a § 16d permit you can work while completing recognition; check your permit's conditions.
No. It is a separate, shorter training under the same act. Apply for the Physiotherapeut title if you are a qualified physiotherapist.
