Physiotherapist jobs in the UK: HCPC registration (2026)
Physiotherapist and physical therapist are protected titles in the UK. If you trained abroad, you need HCPC registration through the international route before you can use either one.
Overview
If you qualified as a physiotherapist abroad, you need HCPC registration before you can use the title physiotherapist or physical therapist anywhere in the UK.
Two physiotherapist assessors read your degree, clinical placements and work history against the physiotherapy standards of proficiency that took effect on 1 September 2023, and either accept you, refuse you or refer you to a test of competence. The scrutiny fee is £678.38 and, if you are accepted, registration usually costs £370.02. OET Physiotherapy is accepted at 1400 overall with no sub-test below 300, alongside IELTS and TOEFL iBT.
This page is for physiotherapists who qualified outside the UK and have never been HCPC registered. That includes graduates of four-year bachelor's programmes, people with a master's or doctor of physical therapy, and experienced clinicians who have since specialised in musculoskeletal, neurological, cardiorespiratory or paediatric practice.
The physiotherapy standards are where most international files succeed or fail. Standard 13.20 expects you to apply manual therapy, exercise and movement, electrotherapeutic modalities and kindred approaches safely, and standards 12.6 to 12.9 expect the underpinning science for the neurological, musculoskeletal, cardiovascular and respiratory systems. Standard 13.14 asks you to form a working diagnosis from your own physiotherapy assessment. Graduates from systems where physiotherapists only treat on a doctor's referral and prescription often under-describe this autonomous assessment and clinical reasoning, and assessors notice.
What makes the UK different is the breadth expected at the point of registration. UK physiotherapy degrees cover musculoskeletal, neurological and cardiorespiratory practice, with placements across hospital and community settings. If your degree or placements were narrow, for example mostly outpatient musculoskeletal work, the course information form and your experience section need to show where the other areas were covered. Gaps in respiratory or acute inpatient practice are a common reason assessors ask for more information.
Once registered, most internationally trained physiotherapists start in the NHS at Agenda for Change Band 5, often on rotations through different specialties, and move to Band 6 specialist posts with experience. NHS Health Careers notes that physiotherapists work in outpatients, women's health, paediatrics and occupational health, as well as in health centres, homes, nursing homes and schools.
Eligibility
- You trained and gained your physiotherapy qualification outside the UK.
- You have never been registered with the HCPC before (if you have, you apply for readmission instead).
- Your qualification is relevant to physiotherapy and is the one that first allowed you to practise in your country.
- Your university or training institution can complete, stamp and sign the HCPC course information form, including theory and practical hours for each course.
- You can show, through training and experience, that you meet every HCPC standard of proficiency for physiotherapists, not only the ones linked to your specialty.
- You have a referee who supervised you or was responsible for you professionally, contactable on an official work email address.
- You meet the HCPC English requirement (IELTS 7.0 with no element below 6.5, the TOEFL iBT scores, or OET Physiotherapy 1400 with no element below 300), unless your qualification is from a listed English-speaking country.
- If you trained in Norway, Iceland, Liechtenstein or Switzerland, a separate Free Trade Agreement route with paper forms applies. Refugees and people with humanitarian protection can contact the HCPC to apply without the scrutiny fee.

Step by step
Check your qualification against the physiotherapy standards
Download the HCPC standards of proficiency for physiotherapists and read them against your degree transcript. Note where respiratory care, neurological rehabilitation, acute care and community practice were taught and assessed. This tells you early whether your application will need strong experience evidence to fill gaps.Book your English test
Unless you are exempt, take IELTS (7.0 with no element below 6.5), TOEFL iBT or the OET Physiotherapy test (1400 with no element below 300). OET is accepted for physiotherapists, which suits many clinicians. If you plan to use one test for the visa as well, IELTS for UKVI is the safer choice. Results must be under two years old when you apply.Send the course information form to your university
The form must be completed by the institution that delivered your primary qualification, with learning outcomes, content, assessment methods and theoretical and practical hours for each course. It must carry the institution's stamp or seal and a real (not typed) signature, and be uploaded as one document, not a scan. Universities can be slow, so start this first.Prepare and certify your documents
You need certified copies of identity and address, your qualification certificate, the English certificate and any change of name document, plus certified translations where documents are not in English. The HCPC asks for documents to be machine-readable. Poorly certified copies are one of the main causes of delay.Write your experience section and mapping document
Describe your professional experience in your own words, most recent first, within 20,000 characters, and explain gaps of six months or more. The HCPC says you must not use AI or copied content, and it checks for both. Your standards of proficiency mapping document should point to where the evidence is, not repeat it.Arrange your referees
Each referee must have supervised you or been professionally responsible for you, and must use an official work email address. The HCPC contacts referees during verification, so warn them and confirm their details are current. Gmail or Hotmail addresses are not accepted.Submit online and pay the scrutiny fee
Complete the online application at hcpcapplication.hcpc-uk.org and pay the £678.38 scrutiny fee by card. Your application is not submitted until the fee is paid. Tell your bank about the international payment first, as declined cards are common.Assessment, and possibly a test of competence
Two physiotherapist assessors review your file and send a decision with a record of assessment. You may be accepted, refused, or offered a test of competence if they need more information. Respond to any request quickly, as missing items after 6 months lead to withdrawal.Verification and registration
After acceptance the HCPC verifies your details with your education provider, overseas regulator and referees, then invites you to pay the registration fee (£370.02 for most international applicants). Your registration number arrives by email. Only then can you call yourself a physiotherapist in the UK.Apply for NHS jobs and your visa
Search NHS Jobs for Band 5 rotational or Band 6 posts. Many employers will interview while your HCPC application is in progress, but you cannot practise as a physiotherapist until registered. Once you have a job offer and certificate of sponsorship, apply for the Health and Care Worker visa.
Exams
HCPC test of competence for physiotherapists
There is no written physiotherapy licensing exam in the UK. If your file leaves a gap, the referral is to a structured professional discussion with two physiotherapists on the Register, built around the specific standards you did not evidence. For a physiotherapist the discussion could cover respiratory assessment and on-call chest physiotherapy, acute neurological rehabilitation, or how you reach a working diagnosis and set timescales in a management plan without a medical referral. Prepare case examples from each of the four core systems and be ready to explain your reasoning, risk management and when you would refer on.
OET Physiotherapy, IELTS or TOEFL iBT
OET runs a physiotherapy version, and only that version counts for this register; a nursing or medicine OET result will not do. Its role-plays use physiotherapy scenarios such as explaining an exercise programme or a mobility plan, which many clinicians find easier than general IELTS writing tasks. IELTS and TOEFL iBT remain options, and IELTS for UKVI is the one result that can also serve the visa.

English
- Physiotherapists need the standard HCPC level: IELTS 7.0 overall with every band at 6.5 or above, OET Physiotherapy at 1400 with each sub-test at 300 or above, or the HCPC's TOEFL iBT scores (for tests from 21 January 2026, Reading 4.5, Listening 5, Writing 4.5, Speaking 5 and Overall 5).
- Only the physiotherapy OET counts. If you sat OET for another profession in an earlier career, you will need to resit in physiotherapy.
- Degrees from India, Pakistan, Egypt, Jordan, the Philippines, Nigeria and the GCC are not on the HCPC list of English-speaking countries, so most applicants on this route need a test even if the whole course was taught in English.
- If your entry-level physiotherapy degree came from Australia, Canada, Ireland, New Zealand, the USA or another listed country, you are exempt from testing, even if you later worked elsewhere.
- Results must be under two years old on the day you submit, so time the test around when your university will return the course information form rather than taking it first.
- OET Physiotherapy Writing and Speaking tasks are set in physiotherapy scenarios, so the language you practise for the test is the language of your daily work with patients and colleagues.
- The Health and Care Worker visa needs CEFR B2 English, a separate rule from the HCPC; full details are on our UK country guide.

Timeline
- The slowest part for most physiotherapists is the university course information form, especially from large private physiotherapy colleges; ask for it before you book OET or IELTS.
- If you need clinical hours from a separate compulsory internship year, request a letter from the hospital that supervised it at the same time, because assessors read the internship as part of your training.
- The HCPC currently says about 3 months from a complete file to a first decision by two physiotherapist assessors, then about 3 months of verification with your university, regulators and referees.
- A referral for a test of competence adds time: it has to be completed within 12 months, and the result comes by email within two weeks.
- Many NHS trusts interview for Band 5 rotational posts while your HCPC application is under assessment, so start applying once you have a first decision date in view.
- Once registered, the time to a sponsored job offer, certificate of sponsorship and visa varies by employer and by each trust's recruitment cycle; GOV.UK says the visa decision itself usually takes about 3 weeks after you prove your identity.
Pay
NHS physiotherapists are paid on Agenda for Change. NHS Health Careers says physiotherapists typically start on Band 5. Figures below are the NHS Employers 2026/27 annual pay points from 1 April 2026; pay in Scotland, Wales and Northern Ireland can differ, and London posts add a high cost area supplement (20% of basic pay in inner London, subject to a minimum of £5,794 and maximum of £8,746).
Band 5 (newly registered, often rotational)
£32,073 to £39,043
Intermediate point £34,592 after 2 years; top after a further 2 years.
Band 6 (specialist physiotherapist)
£39,959 to £48,117
Common for experienced international physiotherapists once in post.
Band 7 (highly specialist or team lead)
£49,387 to £56,515
Band 8a (advanced practitioner or service lead)
£57,528 to £64,750
Employers
- NHS hospital trusts, often through Band 5 rotations across musculoskeletal outpatients, wards, critical care and neurology.
- NHS community and integrated care services, including home visits, intermediate care and nursing homes.
- Musculoskeletal and first-contact services working with GP practices.
- Paediatric services and special schools.
- Private hospitals, sports clinics and independent practices (these may not qualify for the Health and Care Worker visa unless they provide services to the NHS).
- Occupational health providers.
Visa
- Physiotherapists are SOC 2020 code 2221, which GOV.UK lists as eligible for new Health and Care Worker visa applications.
- The visa fee, salary threshold, maintenance funds, surcharge exemption and visa English rule are the same for every profession on this route, so we keep them on our UK country guide, and the HCPC application mechanics on our HCPC regulator page.
- For a physiotherapist, the sponsor is usually an NHS trust or health board recruiting into Band 5 rotations or Band 6 posts, or a private provider that delivers physiotherapy to the NHS, such as a musculoskeletal or first-contact service.
- Sports clubs, gyms and private clinics that only see private patients do not qualify for the Health and Care Worker visa; they would need to sponsor you on the Skilled Worker visa.
- Your going-rate salary is read from GOV.UK's table of national pay scales for eligible healthcare jobs, so a Band 5 physiotherapist offer is assessed against the Band 5 rate for the UK nation you work in.
- If you are offered a physiotherapy assistant or support worker post while waiting for registration, check the occupation code with the employer: it is not code 2221 and may not be eligible.
Moving from the Gulf
- 1
A DHA, DOH, MOHAP, SCFHS, QCHP or other GCC physiotherapy licence does not transfer. You still apply through the HCPC international route, and the HCPC assesses the qualification that first allowed you to practise, not your Gulf licence.
- 2
Gulf hospital work is useful evidence, especially ICU chest physiotherapy, post-operative orthopaedic rehabilitation and stroke rehabilitation. Describe how you assessed patients and set goals yourself, because UK standards expect autonomous diagnosis, and many Gulf posts work on physician referral.
- 3
Referees must be supervisors with official employer email addresses. Ask your head of rehabilitation or physiotherapy lead early, before you leave the job.
- 4
List every Gulf registration in the professional registration section. The HCPC says it contacts relevant regulatory bodies during verification, so make sure your licences are in good standing and that you can request letters of good standing or verification from each authority if asked.
- 5
A Gulf degree or licence does not exempt you from English testing, because GCC countries are not on the HCPC's English-speaking list. If your degree is from an Indian, Egyptian, Filipino or similar university, the same applies.

Questions
No written licensing exam is required. The HCPC assesses your training and experience on paper. If the assessors need more information, they may offer a test of competence, which is a structured professional discussion with two physiotherapist assessors.
Yes. The HCPC accepts the OET Physiotherapy test with an overall score of 1400 and no element below 300. Remember the visa has a separate English rule, so check whether your test also works for UK Visas and Immigration.
Yes. Physical therapist and physiotherapist are both protected HCPC titles, so a US doctor of physical therapy or a Philippine BSPT graduate applies through the same physiotherapy route. US graduates are exempt from English testing; Philippine graduates are not.
Since 1 September 2023 the physiotherapy standards expect knowledge of the neurological, musculoskeletal, cardiovascular and respiratory systems, and practice skills including manual therapy, exercise, electrotherapy and related approaches. They also expect you to form your own working diagnosis and management plan, so describe autonomous assessment, not only treatment on referral.
NHS Health Careers says physiotherapists typically start on Band 5. Experienced international physiotherapists may be appointed to Band 6, but this depends on the employer and the post.
You cannot use the title physiotherapist until you are registered. Some people take support worker roles, but these may not meet visa salary or occupation rules. Check with the employer and GOV.UK before accepting.
It can be. Assessors compare your training against all physiotherapy standards of proficiency. Use your hospital and inpatient experience, and any further training, to show respiratory and neurological competence.
It can. HCPC standard 13.20 for physiotherapists names manual therapy, exercise and movement, electrotherapeutic modalities and kindred approaches as practice skills you must apply safely and effectively. Show where each was taught and assessed on the course information form, and add post-qualification courses and clinical examples in your experience section.
No. The Chartered Society of Physiotherapy describes itself as the professional, educational and trade union body for the UK's chartered physiotherapists, students and support workers, and offers international affiliate membership. Membership is optional and is not a licence: only HCPC registration lets you practise as a physiotherapist.
After Band 5 rotations, most physiotherapists specialise at Band 6 in areas such as musculoskeletal, neurology, respiratory or paediatrics. NHS Health Careers lists sports injuries, critical care, older people, children and cancer care as specialisms, and teaching, research and management as further options.
You can apply for assistant posts if you already have the right to work in the UK, but you must not call yourself a physiotherapist or physical therapist until you are registered. Assistant posts are graded below Band 5 and use a different occupation code, so they rarely support a visa on their own.
No. The NHS Employers code of practice for international recruitment says health and care staff must not be charged recruitment fees for getting a UK job. NHS trusts advertise Band 5 and Band 6 physiotherapy posts on NHS Jobs, and you can apply directly.
