Occupational therapist jobs in the UK: HCPC route (2026)

Occupational therapist is a protected title in the UK. Overseas-trained OTs register with the HCPC, then work across the NHS, social services and other settings where the visa rules can differ.

Overview

Occupational therapist is a legally protected title in the UK, so an OT who trained abroad cannot take a post or call themselves an occupational therapist until the Health and Care Professions Council (HCPC) has added them to its Register.

The direct answer: you need an OT qualification that first let you practise, evidence that you meet the HCPC standards of proficiency for occupational therapists, and English at IELTS 7.0 (no element below 6.5) or OET Occupational therapy 1400 (no sub-test below 300), unless your degree came from a listed English-speaking country. There is no OT licensing exam; two occupational therapist assessors judge your file, and if it leaves gaps they refer you for a test of competence.

What decides most OT applications is occupation itself. The HCPC standards written only for occupational therapists ask you to listen to a service user's occupational narrative and analyse it to plan for the future (standard 7.9), to understand the relationship between person, environment and occupation, to select standardised assessments, to choose therapeutic media and activities, and to formulate needs in occupational terms. Applicants whose degrees were built on a medical or physical rehabilitation model often describe exercises, splints and range of movement well but say little about occupational formulation, activity analysis or occupational science. That is the gap assessors notice.

The second common shortfall is breadth. The occupational therapist standards cover physical health, mental health, learning disability and social care, and expect you to assess health, social and employment needs. If your training or career has been mostly paediatric, hand therapy, sensory integration or private neuro clinics, plan in advance how your course information form, placement record and experience section will show adult mental health and community practice. NHS Health Careers describes OTs helping someone adapt to life after major surgery, supporting people with mental illness or learning disabilities with work or volunteering, and helping older people stay at home with adaptations such as level access showers or stair lifts. Your evidence should look like that range.

This page covers what is specific to occupational therapists. The parts every HCPC profession shares (the online international application, certified documents, the £678.38 scrutiny fee, verification and the test of competence process) are explained on our HCPC regulator page, and the Health and Care Worker visa and NHS pay system on our UK country guide.

The UK is unusual in how much OT work sits outside hospitals. The Royal College of Occupational Therapists (RCOT) lists jobs in the NHS, local authorities, the independent sector and community settings. Council social services posts use local government pay scales rather than Agenda for Change, and whether they can sponsor you depends on the employer, so the setting you aim for shapes both your pay and your visa.

Eligibility

  • You hold an occupational therapy qualification gained outside the UK, and it is the award that first entitled you to practise as an OT.
  • You have never held HCPC registration; former registrants apply for readmission instead.
  • Your course included supervised practice placements that your university can record, with hours, on the HCPC course information form.
  • You can show the OT-specific standards: occupational narrative and formulation, person-environment-occupation reasoning, activity analysis and choice of therapeutic media, and standardised assessment.
  • Your evidence spans physical and mental health and community or social care work, not only your specialty.
  • Your English meets IELTS 7.0 with no part under 6.5, OET Occupational therapy 1400 with no part under 300, or the HCPC TOEFL iBT scores, unless you qualified in a listed English-speaking country.
  • Referees are OTs or managers who supervised you and can be reached on an official work email.
  • You can hold professional indemnity cover once registered; RCOT says this is an HCPC requirement and that RCOT membership includes it.
  • Training or practice in Norway, Iceland, Liechtenstein or Switzerland goes through the separate Free Trade Agreement route.

Step by step

  1. Review the occupational therapy standards

Download the HCPC standards of proficiency for occupational therapists and compare them with your training. Look at occupation-focused assessment, environmental adaptation, mental health and working in social care. Note where your evidence is thin.
  2. Take an accepted English test

IELTS (7.0 with no element below 6.5), TOEFL iBT, or OET Occupational Therapy (1400 with no element below 300) are accepted. Only the occupational therapy version of OET counts. Results must be under two years old when you apply.
  3. Get the course information form from your university

The form must show learning outcomes, content, assessment and theory and practical hours, with the institution stamp and a real signature. Ask the university to list practice placements by setting if possible. It is uploaded as one document.
  4. Certify and translate documents

Prepare certified copies of identity, address, qualification and English certificate, with certified translations if needed. Follow the HCPC's certification and machine-readable guidance to avoid delays.
  5. Write your experience and complete the mapping

Describe your OT practice in your own words, most recent first, up to 20,000 characters, explaining gaps of six months or more. Include home assessments, equipment and adaptation work, discharge planning, and mental health or community work. The HCPC prohibits AI or copied text.
  6. Confirm referees

Referees must have supervised you and use official work email addresses. The HCPC contacts them at verification, so confirm they are reachable.
  7. Submit and pay the scrutiny fee

Apply online and pay the £678.38 non-refundable scrutiny fee. The HCPC aims to give a first update within 10 working days and assessment takes about 3 months from a complete application.
  8. Assessment and possible test of competence

Two OT assessors decide whether you meet the standards. If they need more information, they may offer a structured professional discussion within 12 months, usually online.
  9. Register and find a sponsored post

After verification, pay the registration fee and receive your number. Look for NHS Band 5 or Band 6 OT posts, or adult social care roles with sponsor licences, and apply for the visa once you have a certificate of sponsorship.

Exams

  • Test of competence for occupational therapists

There is no OT licensing exam. If the two OT assessors cannot confirm from your file that you meet every standard, the HCPC refers you for a test of competence, a discussion with occupational therapist assessors focused on the gaps in your Record of Assessment. For OTs that usually means occupation-focused reasoning: how you took an occupational history, analysed an activity, graded it, adapted an environment, and planned discharge or community support. The HCPC says it is normally held online on Microsoft Teams and must be completed within 12 months; our HCPC regulator page covers the logistics.

  • OET Occupational therapy or IELTS

OET Occupational therapy uses OT case notes and role plays with patients, which many OTs find closer to daily work than IELTS. The HCPC accepts only the occupational therapy version of OET for this register, at 1400 overall with no sub-test under 300, taken at a test centre. IELTS Academic or General Training at 7.0, with no band under 6.5, is the alternative.

English

  • OTs need the standard HCPC level, lower than the speech and language therapist level: IELTS 7.0 overall with every band at 6.5 or above.
  • OET is accepted only in its Occupational therapy version, at 1400 overall with every sub-test at 300 or above. A nursing or medicine OET result does not count for this register.
  • TOEFL iBT is also accepted at the HCPC scores for its non-speech professions; check the HCPC English page for the scale that applies to your test date.
  • Home or remote editions of any test are not accepted, and results must be under two years old when you apply.
  • No test is needed if the OT qualification that first let you practise came from a country the HCPC lists as predominantly English-speaking, such as Australia, Canada, Ireland, New Zealand or the USA. OT degrees from India, the Philippines, the Gulf or Egypt do not qualify for that exemption, even if taught in English.
  • Exemptions, one-skill retakes and the separate visa English rule are explained on our HCPC regulator page and UK country guide.

Timeline

  • Start with the course information form: OT programmes with long placement blocks in several hospitals often need the university to collate hours from different departments, which can take weeks.
  • Booking OET Occupational therapy depends on test centre dates in your country; allow time for a resit if writing or speaking falls short of 300.
  • The HCPC currently gives about 3 months from a complete application to a first decision by the OT assessors, longer when application numbers are high.
  • If you are referred for a test of competence, plan for the session and two weeks for the result, within the HCPC's 12 month limit.
  • Verification follows acceptance; the HCPC contacts your university, regulators and referees, so warn your former OT manager.
  • Allow six to nine months from first document request to an HCPC registration number, then the visa stage described on our UK country guide.

Pay

NHS Health Careers says occupational therapists typically start on Agenda for Change Band 5. Figures are NHS Employers 2026/27 annual pay points from 1 April 2026. Local authority OT posts use council pay scales instead, which differ by council. NHS pay in Scotland, Wales and Northern Ireland can also differ.

  • Band 5 (occupational therapist)

    £32,073 to £39,043

  • Band 6 (specialist occupational therapist)

    £39,959 to £48,117

  • Band 7 (highly specialist or team lead)

    £49,387 to £56,515

  • Band 8a (advanced practitioner or service lead)

    £57,528 to £64,750

Employers

  • NHS acute hospitals (wards, stroke, orthopaedics, discharge teams).
  • NHS mental health trusts, inpatient and community.
  • NHS community and intermediate care teams.
  • Local authority social services (check visa eligibility, as councils are not the NHS).
  • Adult social care providers with sponsor licences.
  • Charities and prison healthcare services.

Visa

  • Occupational therapists are SOC 2020 code 2222 on the GOV.UK eligible occupations list, rated higher skilled, and the code can be used for the Health and Care Worker visa.
  • Occupational therapy assistant roles fall under a different code, 6131 (nursing auxiliaries and assistants), so a support post while you wait for HCPC registration does not carry your OT code.
  • NHS trusts and adult social care providers are typical sponsors. Council social services, schools and some charities employ many OTs, so ask any non-NHS employer whether it holds a sponsor licence and which visa route the post fits before you accept.
  • The visa rules that apply to every health profession (salary threshold, fees, surcharge exemption, English at B2, dependants) are on our UK country guide, and the HCPC route itself on our HCPC regulator page.

Moving from the Gulf

  1. 1

No GCC OT licence, whether DHA, DOH, MOHAP, SCFHS or QCHP, transfers to the HCPC; assessors look at the degree that first let you practise and your experience against the OT standards.

  2. 2

Much Gulf OT work is in hospital rehabilitation, private paediatric centres or sensory integration clinics. Add evidence of adult mental health, home environment assessment and community practice, which UK OTs are expected to cover.

  3. 3

Describe Gulf work in occupational terms: the occupations clients wanted to return to, how you analysed and graded activities, and how culture, family roles and home layout shaped your plan.

  4. 4

Gulf qualifications are not on the HCPC English-speaking list, so plan for OET Occupational therapy at 1400 or IELTS 7.0 even if you have worked in English for years.

  5. 5

Ask your Gulf rehabilitation manager to act as referee on an official hospital email before you leave, and request letters of good standing from each GCC authority you were licensed by.

Questions

Yes, but only the Occupational therapy version of OET, at 1400 overall with no sub-test below 300, taken at a test centre. IELTS 7.0 with no band below 6.5 is the alternative.

No. Occupational therapist assessors review your file on paper. If evidence is missing against the OT standards, the HCPC refers you for a test of competence rather than asking for more documents.

Usually because the file shows clinical rehabilitation but not occupation-focused practice: occupational narrative, activity analysis, environmental adaptation and occupational formulation, which the HCPC standards for OTs single out. Thin evidence in mental health or community practice is the other common gap.

It can be. The OT standards are not age-specific, so show where your course covered adults, mental health and social care, and use any placements or jobs in those areas in your experience section.

NHS Health Careers says occupational therapists typically start on Agenda for Change Band 5, which is £32,073 to £39,043 in England in 2026/27. Specialist posts are Band 6 and team leads usually Band 7.

Only if it holds a sponsor licence and the post fits a visa route. The Health and Care Worker visa needs an NHS, NHS-contracted or adult social care employer, so ask the council which route it uses before accepting.

You cannot use the title occupational therapist until you are registered. Assistant posts sit under a different occupation code (6131), so any visa would be judged on that role, not on your OT qualification.

No. The HCPC assesses your own qualification and experience against its standards of proficiency; no international accreditation gives automatic registration, so your evidence still has to cover every standard.

Yes. RCOT says that to practise you must meet the HCPC's requirements, which include professional indemnity, and that RCOT membership includes indemnity insurance. Check what your employer covers and arrange your own for private work.

Most OTs should allow six months or more for the HCPC stage, longer if the university is slow with the course information form or a test of competence is needed. The visa usually follows in weeks once you have a sponsored job.

No. The NHS code of practice for international recruitment, updated in March 2025, says international health and social care staff must not be charged recruitment fees for getting a UK job.
