Iscrizione all'albo (registration with an Italian Ordine)
Definition
Iscrizione all'albo is registration in the professional register (albo) kept by your provincial Ordine, such as the OMCeO for doctors and dentists or the OPI for nurses. In Italy it is the step that lets you practise after the Ministry of Health has recognised your foreign qualification.
Also called: iscrizione all'albo, albo professionale, Ordine dei Medici, OMCeO, OPI, albo infermieri, iscrizione OPI

What is the albo?
Italian health professions are self-regulated through provincial Ordini, coordinated nationally by federations such as FNOMCeO (doctors and dentists) and FNOPI (nurses). Each Ordine keeps the albo for its province. As OPI Verona summarises, the law decides which professions need registration in an albo, and the Ordini check the requirements, keep the registers and discipline members under state supervision.
For health professions that have no Ordine, article 50 of DPR 394/1999 provides special lists (elenchi speciali) kept by the Ministry of Health, and the ministry also publishes each year the list of foreign nationals whose qualifications it has recognised.
You usually register with the Ordine of the province where you live or are domiciled; OPI Verona, for example, requires residence or domicile in Verona or its province.
The extra steps for foreign-trained professionals
- Recognition. Obtain a decreto di riconoscimento from the Ministry of Health.
- Language and rules test. Article 50 of DPR 394/1999 makes registration of foreign-qualified professionals conditional on a check of their Italian and of the special rules on practising in Italy, carried out by the Ordine before registration and paid for by the applicant. The decree states whether you sit Italian only or Italian plus professional rules.
- Know which test applies. OPI Verona explains that EU citizens sit an Italian language exam, while non-EU citizens sit Italian and the rules of nursing practice when their decree requires both.
- Apply to register with the Ordine's form, by certified email (PEC) or in person, and pay the fees.
- Wait for the board to approve your registration at its next meeting.
Deadlines and fees
Article 50 also says you must register within two years of the date of your recognition decree; after that the decree loses its effect and you must apply to the ministry to renew it before going back to the Ordine.
Fees are set locally. Two examples:
- OMCeO Milano (doctors): registration fee €135 (€70 for new graduates), government concession tax €168 and a €16 revenue stamp. Foreign-qualified doctors also sit a test there on the Italian needed to write medical certificates and on the rules of practice.
- OPI Verona (nurses): €100 to sit the Italian, or Italian and nursing regulation, exam before applying to register.
Common problems
- Letting the decree lapse. Two years pass quickly while you wait for a job or a test session.
- Applying to the wrong Ordine. Check the residence or domicile rule of the province first.
- Confusing the derogation with registration. Decree-law 34/2023 allows temporary work until 31 December 2029 in derogation from articles 49 and 50 of DPR 394/1999, through regional procedures. That route is tied to the region and employer and is not the same as entering the albo.
- Weak Italian. The Ordine test is practical and job-focused, so prepare with medical or nursing Italian, not only general courses.
Questions
Yes, on the normal route. After the Ministry of Health recognises your qualification you register in the albo of your provincial Ordine. The regional derogation under Decree-law 34/2023 is a separate, temporary exception.
Article 50 of DPR 394/1999 requires the Ordine to check your Italian and your knowledge of the rules for practising in Italy before registration. Your recognition decree says which parts apply to you.
Two years from the date of the decree. After that the decree loses its effect and the Ministry of Health has a procedure to renew it.
It varies by Ordine. OMCeO Milano lists €135 (or €70 for new graduates) plus a €168 government concession tax and a €16 revenue stamp; OPI Verona charges €100 to sit its language exam.
Normally the one for the province where you live or are domiciled. OPI Verona, for example, requires residence or domicile in its province.
Yes. The Ministry of Health says EU, EEA and Swiss citizens with a qualification obtained outside the EU must also apply for recognition before registering with an Ordine.
