About Alexandrovska

University Hospital Alexandrovska (UMBAL Alexandrovska EAD) was founded in 1879 as Sofia's first-class hospital and was opened in its own buildings, named after Prince Alexander I, in 1884.

From its foundation it had the status of a national medical institution, and it hosted the exams in which doctors who graduated abroad proved their right to practise in Bulgaria.

The hospital describes itself as the first university and multi-profile hospital in Bulgaria and the largest in the country. It runs 26 independent clinics, three reference laboratories and many departments and centres, and it is the main clinical base of the Medical University of Sofia, which trains Bulgarian and foreign students there.

It lists among its strengths organ and cell transplantation, including what it calls Bulgaria's most developed kidney transplant centre, invasive cardiology, neuronavigation, nuclear medicine with a PET/CT centre, and centres for rare neurological and nephrological diseases and primary immune deficiencies.

For an internationally trained professional, Alexandrovska is a prestigious academic employer, but it hires on Bulgarian terms. Non-EU graduates must pass the Bulgarian medical language exam and the Article 186 exam before they can practise, and its adverts ask for applications in Bulgarian.

What it is known for

  • Kidney transplantation

The hospital describes the Center for Kidney Transplants at its Clinic of Urology as the most developed in Bulgaria, and its Clinic of Nephrology and Transplantation prepares living related donors and follows transplant patients.

  • Clinical immunology and stem cell bank

A Clinic of Clinical Immunology with a stem cell bank, home to the hospital's expert centre for primary immune deficiencies.

  • Neurology

A Department of Neurology with what the hospital calls the country's largest laboratory for ultrasound study of the nervous system and brain vessels, and a centre for rare neurological diseases.

  • Invasive cardiology

A Center for Intravascular Interventional Procedures at the Clinic of Cardiology.

  • Nuclear medicine

A PET/CT centre at the Department of Nuclear Medicine; the hospital announced FAPI PET/CT for cancer diagnosis in 2026.

  • Ophthalmology

An eye clinic with laser equipment for screening and refractive surgery, including equipment the hospital says was the first in Bulgaria for treating children under anaesthesia.

  • Endocrinology

A Center for Prevention of Type 2 Diabetes and its Complications at the Clinic of Endocrinology.

International recruitment

  • Alexandrovska does not publish an international recruitment programme, relocation help or visa sponsorship offer, and it names no recruitment agency. Its careers page is in Bulgarian.
  • The careers page invites people to submit an application and CV to the human resources department, in person at the hospital administration or by email. Individual adverts name their own contact; an advert on the page in October 2026 for nurses in a new paediatric urology department asked for higher or semi-higher medical education and said previous experience was an advantage but not required.
  • That nurse advert describes pay as competitive with the highest nurse pay in Sofia and the country, with paid overtime and night work, supplements for specialty and qualifications, 21 to 25 days of basic annual leave plus 8 days of additional leave, and training. It states no figure.
  • International applicants usually reach the hospital after completing Bulgaria's national route: the Bulgarian language and terminology exam, the Ministry of Health's admission order and the university exam under Article 186 of the Health Act, then registration with the Bulgarian Medical Association or the Bulgarian Association of Health Professionals in Nursing.

What you need before you can be hired

  • The Article 186 route for non-EU graduates. Foreigners with a medical qualification from a third country practise only after proving Bulgarian and professional terminology and passing an exam that includes the state examinations Bulgarian graduates sit. The Minister of Health admits you to the exam by order, and a Bulgarian university runs it twice a year; you must sit it within one year of the order.

  • Bulgarian language and terminology. The exam under Ordinance No. 15 of 2005 is run by the Institute of Education in Sofia. No CEFR level is published; the exam is profession-specific.

  • Registration. Doctors join the register of a regional college of the Bulgarian Medical Association (BLS); nurses and midwives join the Bulgarian Association of Health Professionals in Nursing (BAPZG).

  • Work and residence permit. Your employer applies to the Employment Agency for a single permit or EU Blue Card approval; you then apply for a type D visa and collect the permit from the Migration Directorate. A labour market test of 15 days applies to the single permit.

  • EU qualifications. EU, EEA and Swiss citizens and holders of EU qualifications use recognition under the Recognition of Professional Qualifications Act instead of the state exams, though non-EU citizens still need the language certificate.

How to apply

  1. Pass the Bulgarian medical language exam

Register with the Institute of Education in Sofia for the exam in Bulgarian and professional terminology under Ordinance No. 15.
  2. Apply to the Ministry of Health for admission

File the application for admission to the Article 186 exam with legalised, translated documents. The Ministry says no fee is charged to examine it.
  3. Sit the university exam

Take the exam at the Bulgarian university named in the Minister's order within one year, then collect the rector's certificate.
  4. Register with your professional body

Doctors join a regional college of the BLS; nurses and midwives join BAPZG.
  5. Apply to the hospital

Send an application and CV to the human resources department at the hospital administration or by the email on the careers page, or to the contact named in a specific advert.
  6. Arrange your permit

If you are not an EU citizen, the hospital applies to the Employment Agency for your single permit or EU Blue Card before you apply for a type D visa.

Pay

Bulgaria has no national pay table for doctors or nurses that we could confirm for 2026. Salaries are set by each employer within the framework of the healthcare sector collective labour agreement; the latest one on the Ministry of Health's site dates from April 2022. Alexandrovska publishes its own collective labour agreement, most recently dated 1 March 2026, but we could not extract pay figures from it, and its adverts do not state salaries.

Questions

It has no published international programme. Non-EU doctors can work there only after passing Bulgaria's language exam and the Article 186 exam and registering with the Bulgarian Medical Association. Nurses and other health professionals trained outside the EU also need Bulgarian recognition and membership of their professional body, such as BAPZG for nurses.
